Case-control Study On The Phenomenon And Regularity Of Acupoint Hyperalgesia In Colorectal Cancer

Abjective:To explore the regularity and changes of acupoint pain sensitization in patients with colorectal cancer(CRC),to quantitatively observe the mechanical pain threshold of CRC related acupoints in pathological state,to define the occurrence of pain sensitization in CRC patients,and to explore the influence of related factors on pain sensitization.Methods:In this study,a case-control study was used to select CRC patients from four hospitals in Chengdu as the case group,healthy people with similar sex and age served as control group,the ratio of the case group to the healthy control group was 1:2,and the ratio was adjusted by pass15 software.The basic information of the two groups was collected through one-to-one interview questionnaire,and the mechanical pain threshold of body surface acupoints was used as the observation index,electronic von Frey was used to measure and record the mechanical pain threshold information of 16 acupoints on the body surface.Epi Data database and SPSS software were used for data entry and analysis.Results:1.A total of 332 subjects were included,including 134 CRC patients and 198 healthy subjects.2.The mechanical pain threshold values of related acupoints in CRC patients were lower than those in healthy people(P<05.0),the mechanical pain thresholds of four parts in CRC patients were lower than those in healthy people(P<05.0),the mechanical pain threshold of each meridian in CRC patients was lower than that in healthy people(P<05.0).3.After drawing ROC curve of all the mechanical pain threshold values of acupoints in CRC patients and healthy people,the cut-off values of most acupoints(9 points)were relatively well defined,with area under the curve > 0.7 and Youden index > 0.3,the sensitization rate of acupoint pain in CRC patients was higher than that in healthy people(P<05.0).The ROC curve of mechanical pain of each parts threshold was drawn,the area under the curve was more than 0.7 and Youden index was more than 0.38,the pain sensitization rate of patients with various parts was higher than that of healthy people(P<05.0).After drawing ROC curve of mechanical pain measurement value of 7 meridians,the cutoff value of most meridians(6 meridians)was well defined,with area under the curve > 0.7 and Youden index > 0.37;the pain sensitization rate of patients with various meridians was higher than that of healthy people(P<05.0).4.The mechanical pain threshold and pain sensitization rate of CRC patients were different among the following groups: The mechanical pain measurement values of Zhongwan,Quchi,Hegu and Daheng points were statistically significant in different course of disease(P<05.0),the difference of mechanical pain sensitization rate of Hegu and Shangjuxu points was statistically significant(P<05.0).There were significant differences in the measurement of mechanical pain in Zhongwan,Fujie and Xiajuxu points with or without chemotherapy(P<05.0),the difference of pain sensitization rate of Zhongwan and other 8 acupoints was statistically significant(P<05.0).5.After logistic regression analysis,it was found that the shorter the course of disease,the more sensitized Hegu Point was,and the more sensitized Quchi point was compared with male patients;the higher the SDS score was,the more sensitized five acupoints such as Quchi were;the higher the KPS score was,the more sensitized Yinlingquan and Dachangshu were,and the more sensitized six acupoints such as Xiajuxu were in the case of chemotherapy.The above results were statistically significant(P<05.0).Conclusions:1.There was pain sensitization in the relevant acupoints of CRC patients,and the pathological information can be expressed through the relevant acupoints.2.The mechanical pain threshold of acupoints in CRC patients was lower than that in healthy subjects,and the pain sensitization rate was higher than that in healthy subjects.The mechanical pain threshold of four parts and seven meridians in CRC patients was lower than that in healthy subjects,and the pain sensitization rate was higher than that in healthy subjects.3.Gender,course of disease,chemotherapy,SDS,KPS may be the influencing factors of acupoint pain sensitization in CRC patients.There is no direct correlation between age,TNM stage,location of CRC lesion,tumor metastasis and pain sensitization,which needs further verification.
