BackgroundChina has the highest level of incidence of cerebral apoplexy in the world which is still rising with an average annual increase of about 6.5%.At present,cerebrovascular disease has become the first cause of death of Chinese residents as well as the single disease with the highest disability rate.There are more than 11 million patients suffering from stroke,of whom about 2.4 million people new stroke occur and 1.1 million people die of stroke every year.In short,cerebrovascular disease has become the main threat to the health of the population in China.ObjectiveThis study,using China information system of disease prevention and control of the A county from 2008 to 2016 residents of cerebrovascular disease death data,aims to:1)Analyze the status and the change trend of the population in A county during2008-2016.2)Analyze the status and the change trend of the mortal mortality in A county during2008-2016,especially caused by cerebrovascular disease.3)Explore the risk factors leading to stroke,thus to put forward corresponding suggestions to improve the region’s population health.MethodsTaking A county in Shandong province as the research site,this study collected birth information,population information,cause of cerebrovascular diseases in death system during 2008 to 2016.All cases of cerebral apoplexy deaths in 2016 in this county were retrospectively investigated using the questionnaire developed by the county center for disease control and prevention,which included basic information of cerebral apoplexy deaths whether they were suffered from hypertension,high diabetes,hyperlipidemia or heart disease,and risk factors of cerebral apoplexy such as smoking and drinking.Data using Microsoft Excel 2016 and SAS9.2 software to analyze and Chi-square test,the mortality rate among different ages and genders,as well as the mortality rate,the change trend and risk factors of cerebrovascular diseases.Results1.The overall burden coefficient of the county is mainly affected by the burden coefficient of children,showing a slow rising trend.The average burden coefficient is30.58%,lower than 50%,meaning that the county is still in the "population opportunity window" period.2.The top three causes of death of residents in this county are all chronic diseases,including cardiovascular and cerebrovascular diseases,tumors and respiratory diseases,accounting for over 80% of all deaths.The first cause of death was cardiovascular and cerebrovascular diseases,with an average crude mortality of 342.17/100,000,accounting for 48.80% of all deaths.After standardization,the average mortality of cardiovascular and cerebrovascular diseases was 257.36/100,000,accounting for 47.53% of all deaths.The mean crude mortality of cerebrovascular disease was 168.88/100,000,and the standardized mortality was 128.31/ 100,000.The crude mortality rate of cerebral hemorrhage was92.0/100,000.The mean crude mortality of cerebral infarction was 50.79 per 100,000.The mortality of cardiovascular and cerebrovascular diseases in males was higher than that in females(p < 0.01).3.In 2016,the mortality rate of cerebral apoplexy in the county was 149.52/100,000,accounting for 20.77% of all causes of death.The age of death was 67.53±10.37 years old,which was lower for males than females.30.97% dead in winter.The age group of stroke death was concentrated at 60~79 years old,accounting for 58.20%.Both males and females experience a sharp increase in stroke deaths after 50 years.4.The top three stroke risk factor of exposure were hypertension,diabetes and smoking before the disease,respectively 79.87%,36.43% and 34.07%.The risk factors of hypertension were significantly higher than other risk factors,and the difference was statistically significant(p < 0.01).The exposure rate of risk factors was different between genders(p < 0.01)and among different age groups(p < 0.01).Conclusions1.The first cause of death of residents in this county is cardiovascular and cerebrovascular diseases,causing higher mortality rate than the national average level.2.The mortality rate of cardiovascular and cerebrovascular diseases and cerebral apoplexy is different among ages and genders which it is higher in the age group over 40 years old,especially over 60,and higher in males than it in females.3.Hypertension,diabetes and smoking before the disease are the main risk factors for stroke,while there are differences in risk factors between different gender and age groups.
