| Objective(1)To understand the current situation of grief reaction and stress disorder in the family members of patients died in Chinese hospitals,analyze the influencing factors of grief response and stress disorder in bereaved patients,and screen out high-risk groups that need to be paid attention to,so as to provide guidance for bereavement intervention.(2)To investigate the painful events experienced by the bereaved at the time of death,and analyze the most unacceptable painful events for the caregivers,so as to provide support for reducing the psychological pressure and mental burden of the bereaved.MethodsThis study investigated the grief reaction and stress disorder(after half a year)of the family members of the patients who died after clinical treatment in a comprehensive tertiary hospital from July 1,2019 to June 30,2020.This study completed the survey by filling in a questionnaire.The questionnaire is divided into three parts.The first part is the sociological data of the bereaved.The second part is used to evaluate the grief reaction and stress disorder of the bereaved after patients’death,which are evaluated by Inventory of Complicated Grief(ICG)and Psychometric properties of the Impact of Event Scale—Revised(IES-R).The third part is to investigate the painful experience of the bereaved in the last few days before the patients’ death.Statistical software SPSS 22.0 was used to analyze the collected data.Taking the ICG score and IES-R score of the bereaved as dependent variables respectively,the causes of patients’ death,the places of patients’ death,relationship between the bereaved and the patients,the bereaved people’s gender,age,marital status,education level,occupation,number of previous funerals,previous contact with others’death and their personality characteristics were used as independent variables for univariate analysis.Among them,if the independent variable was a dichotomous variable,the t-test of two independent samples was used for univariate analysis;if the independent variable was a multi-categorical variable,one-way ANOVA was used for univariate analysis.At the same time,for the statistically significant independent variables in univariate analysis,multiple linear regression analysis was used for multivariate analysis.Among them,the two classification variables directly entered the analysis after assignment,and the multi classification disordered variables entered the analysis after assignment of virtual variables.For all statistical results,when the bilateral p value is less than 0.05,the difference is considered to be statistically significant.ResultsA total of 93 bereaved persons were included in this study,including 71 males,accounting for 76.3%,86 married persons,accounting for 92.5%,and 57 children of dead patients,accounting for 61.3%.The ICG score of the bereaved was 22.013 ±6.385,of which 29 were diagnosed as CG,accounting for 31.2%.Univariate analysis showed that there were significant statistical differences in the place of death,cause of death and the relationship between bereaved and dead patients on the grief reaction of the bereaved.The results of multivariate analysis showed that the relationship between bereaved and dead patients was conjugal relation,and the cause of death of hospital patients was trauma,which was significantly correlated with the occurrence of bereaved grief response.At the same time,the score of IES-R of bereaved persons was 28.675±5.938.35 persons were diagnosed with PTSD,accounting for 37.6%,of which 23 were diagnosed with CG at the same time.Univariate analysis showed that the place of death,cause of death,marital status of the bereaved and the relationship between the bereaved and the dead patients had significant statistical differences on the stress disorder of the bereaved.The results of multivariate analysis showed that the death of patients occurred in ICU,the relationship between bereaved and dead patients was conjugal relation,and the state.of divorce or widowhood was significantly correlated with the occurrence of stress disorder.In addition,in the survey of painful events experienced by caregivers when patients are dying,"no way of saying goodbye to patients any more" is the most painful event for caregivers.ConclusionThe place of death,the cause of death,the relationship between the bereaved and the deceased patients,and the marital status of the bereaved are independent predictors of grief reaction and stress disorder.Among them,when the cause of death of hospital patients is trauma,the death of patients occurs in ICU,the relationship between bereaved and deceased patients is conjugal relation,and the bereaved is divorced or widowed,the bereaved is more likely to have grief reaction and stress disorder. |
