| Objectives Hepatocellular Carcinoma(HCC)is highly invasive and has a high postoperative recurrence rate,among which Microvascular invasion(MVI)is the key factor for the early recurrence of HCC.By retrospectively analyzing the clinical medical records of HCC patients,the high-risk factors of HCC combined with MVI were analyzed,and the possibility of MVI was predicted,so as to assist clinicians in selecting appropriate surgical methods before operation and formulating the best clinical treatment plan,so as to improve the survival rate and the prognosis of patients.Methods A total of 60 patients with hepatocellular carcinoma from the Department of Hepatobiliary Surgery of Hebei Provincial People’s Hospital from January 1,2017 to December 31,2017 were collected as clinical investigation subjects.According to the postoperative pathological report,the patients were divided into MVI negative group and MVI positive group.Clinical data of the patients were collected and univariate analysis was conducted.Multivariate Logistic regression analysis was performed on the important data in univariate analysis: AFP(alpha fetoprotein),neutrophil/lymphocyte ratio(NLR),and tumor diameter to determine the independent risk factors for MVI.Finally,the receiver operating characteristic curve was used to analyze the ability of AFP,NLR and tumor diameter to predict HCC complicated with MVI.Results According to univariate analysis,AFP(P=0.038),tumor diameter(P=0.001)and NLR(P=0.014)were significantly different between the MVI positive group and the MVI negative group.Multivariate Logistic regression analysis showed that NLR(OR:1.296,95%CI:1.157-3.080,P=0.011),tumor diameter(OR:1.296,95%CI:1.064-1.579,P=0.010),AFP(OR:1.002,95%CI:1.000-1.003,P=0.049)were independent risk factors for MVI.The area under the ROC curve of NLR,tumor diameter and AFP prediction of HCC combined with MVI were 0.693,0.783 and 0.661,respectively.Conclusions 1 Preoperative serum NLR level,tumor diameter,and preoperative serum AFP content are independent risk factors for HCC complicated with microvascular invasion.2 The tumor diameter of HCC patients,preoperative serum AFP content and preoperative NLR level were positively correlated with the possibility of developing MVI in HCC patients.3 The effect of tumor diameter in predicting HCC complicated with microvascular invasion was better than that of AFP and NLR.Figure 2;Table 3;Reference 161... |
