Abstract/Summary:
ObjectivesTo construct a theoretical model for the use intention of mobile health services and its influencing factors in patients with gout based on the Health Belief Model and the Information System Success Model, and explore the intention of gout patients to use mobile health services and the influence of patients’health beliefs and information system quality on patients’willingness to use mobile health services through verifying the model. So as to provide theoretical guidance for optimizing the mobile health platform for gout patients and improving patients’willingness to use it.MethodsThis study was a cross-sectional survey, totally 380 patients with gout who attended the outpatient service in Shandong Provincial Gout Clinic Medical Center from October 2019 to January 2020 were selected by convenient sampling, and investigated with a self-designed general information questionnaire and questionnaire of gout patients’intention to use mobile health services and its influencing factors. The latter questionnaire was designed through literature review, group discussion, expert consultations, patient pre-survey and reliability and validity test, including Perceived Susceptibility,Perceived Severity,Perceived Benefits,Perceived Barriers,Self-efficacy,Information Quality,System Quality,Service Quality and Use Intention dimensions.Using the SPSS26.0 software to set up database and input research data,SPSS26.0 was used for statistical analysis, descriptive analysis was used to describe the basic situation of the patients, the use of mobile health services, the scores of the use intention and the variables in the model. To compare the willingness to use mobile health services among patients with different characteristics, the independent sample t-test or univariate analysis was used.Multiple linear regression analysis was used to explore the influence of patients’basic characteristics on their use intention.Pearson correlation analysis was used to analyze the correlation between the variables in the model and use intention. The structural equation model was established by AMOS23.0 to test the theoretical model, and the action path of the variables in the model on the use intention was analyzed.ResultsAccording to the inclusion and exclusion criteria,a total of 380 questionnaires were distributed and 368 completed questionnaires were obtained.Finally,346questionnaires were usable, and the effective recovery rate was 91.05%.There were only 4 female patients in the usable questionnaires,so they were not included in the follow-up analysis. The final usable questionnaires included in this study were 342male patients.1.The form of mobile health used by patients with gout was mainly based on We Chat platform,a total of 42.98%of patients with gout used gout We Chat official account and 16.08%of patients with gout used We Chat Mini Program. Most of the patients’satisfaction with the mobile health services were"a little satisfied"and"satisfied",accounting for 52.34%and 30.12%respectively.The frequency of using mobile health services was concentrated in"not used for a week"and"1-3 days a week",accounting for 56.43%and 38.01%respectively.The commonly used and wished functions by more than half of the patients were diet guidance, disease knowledge and exercise guidance, accounting for 62.65%,56.76%and 51.18%respectively.2.The score of use intention of mobile health services in patients with gout was(3.63±1.07).The scores of each variable of the theoretical model were as follows:Perceived Susceptibility(3.57±1.09),Perceived Severity(3.63±1.17),Perceived Benefits(3.43±1.14),Perceived Barriers(2.59±1.03),Self-efficacy(3.42±1.07),Information Quality(3.62±1.01),System Quality(3.47±1.02),Service Quality(3.52±1.10).3.Univariate analysis showed that there were statistically significant differences in the use intention score of patients with different education level, occupational status, personal monthly income and whether have comorbid diseases(P(27)0.05).Further multiple linear regression analysis showed that the influencing factors of gout patients’intention to use mobile health services included education level, occupational status, personal monthly income, place of residence and whether have comorbid diseases(R~2=0.110,P(27)0.05).4.The results of Pearson correlation analysis showed that except for Perceived Barriers,Perceived Susceptibility,Perceived Severity,Perceived Benefits,Self-efficacy,System Quality,Information Quality and Service Quality were positively correlated with use intention.According to the structural equation model analysis, the Perceived Susceptibility,Perceived Severity,Perceived Benefits,Information Quality,System Quality and Service Quality positively affected gout patients’intention to use mobile health services(P(27)0.05).While Perceived Barriers negatively affected the intention to use mobile health services in patients with gout(P(27)0.05).The mediating effect test results of Bootstrap self-help sampling method showed that the Information Quality also indirectly affected gout patients’use intention through Perceived Benefits. The variables included in the theoretical model had explained 56%of the variance of gout patients’intention to use mobile health services.Conclusions1.Patients with gout had mainly used We Chat platform to obtain mobile health services, patients’satisfaction with the use of mobile health services and use frequency were not high.Patients used mobile health services mainly to obtain gout-related knowledge and lifestyle guidance.2.Patients with gout had the intention to use mobile health services, the use intention was at a moderately high level.3.There were differences among different gout patients with different education level, occupational status, personal monthly income, place of residence and whether have comorbid diseases.4.Gout patients’Perceived Susceptibility,Perceived Severity,Perceived Benefits, and the Information Quality,System Quality, and Service Quality of the mobile health platforms affected the use intention positively, while Perceived Barriers negatively affected the use intention, and the Information Quality also affected patients’use intention through Perceived Benefits.
