Prognostic Significance Of Preoperative Neutrophil-to-lymphocyte Ratio And Platelet-to-lymphocyte Ratio In Primary Fallopian Tube Carcinoma

Abstract/Summary:
Objective:Primary fallopian tube carcinoma(PFTC)is a rare malignant tumor of the female reproductive system.Preoperative diagnosis rate is low,easy to be misdiagnosed,and the prognosis is poor.This study is to investigate the relationship between the neutrophil-to-lymphocyte ratio(NLR)and the platelet-to-lymphocyte ratio(PLR)in the peripheral blood of patients with PFTC before surgery and clinical pathological features,and to explore the predictive value of these indicators in the prognostic evaluation of PFTC.Methods:In this study,108 cases of patients with PFTC admitted to in the Department of Obstetrics and Gynecology of Affiliated Hospital of Qingdao University from March 2005to December 2018 were analyzed retrospectively.Clinical data collection:age,menopausal status,fertility history,clinical symptoms,surgical procedure,lymph node involvement,histological type,tumor differentiation degree,FIGO stage,preoperative CA125 level,routine blood test,NLR and PLR were calculated.Overall survival(OS)and progression-free survival(PFS)were calculated based on postoperative follow-up results.The SPSS22.0 software was used for statistical correlation analysis and processing.By drawing receiver operating characteristic(ROC)curve,the optimal cut-off values of NLR and PLR were determined according to Youden index.NLR and PLR were divided into high and low groups on the basis of the cut-off.The correlation between different NLR groups,PLR groups and clinicopathological features was compared byχ~2 test.Kaplan-Meier method was used to draw the survival curve for comparing the survival of patients with PFTC in different NLR groups and PLR groups,and log rank test was performed.Univariate and multivariate prognostic analysis was performed by Cox regression model.When P<0.05,the difference between the groups was statistically significant.Results:According to the collected medical records,ROC curve was drawn by SPSS 22.0software,and the study groups were as follows:high NLR group(42 cases),low NLR group(66 cases),high PLR group(32 cases),low PLR group(76 cases).By comparing the relationship between NLR,PLR and clinicopathological features of patients with PFTC,the study showed that preoperative peripheral blood NLR and PLR before operation were correlated with FIGO stage in patients with PFTC(P<0.05),but not with other clinicopathological features.Kaplan Meier survival curve analysis showed that the median survival time of 108 patients was 65 months.The 3-year and 5-year survival rates were70.2%and 52.8%,respectively.The median survival time of patients with FIGO stage I-II was 81 months,and the 3-year and 5-year survival rates were 90.5%and 66.7%,respectively;The median survival time of patients with FIGO stageⅢ-Ⅳwas 52 months,and the 3-year and 5-year survival rates were 67.6%and 46.7%,respectively.The clinical survival prognosis of high NLR group were obviously shorter than that of low NLR group;The clinical survival prognosis of high PLR group was apparently shorter than that of low PLR group(P<0.05).Cox regression analysis was used to analyze the corresponding clinicopathological factors of patients with PFTC:univariate analysis showed that FIGO stage,NLR and PLR were correlated with the prognosis of patients with PFTC,and the results were statistically significant(P<0.05).Multivariate analysis showed that FIGO stage,preoperative NLR were independent risk factors for the prognosis of patients with PFTC.Conclusion:1.The survival time of patients with high NLR and PLR levels was shorter than that of patients with low NLR and PLR levels.2.NLR and FIGO stage were independent risk factors for the prognosis of patients with PFTC.The increased levels of NLR and PLR may be associated with poor prognosis of patients with PFTC.3.In terms of monitoring tumor inflammatory response,NLR combined with PLR could predict the prognosis of patients with PFTC.
