ObjectiveThis study intends to use retrospective research methods to collect the general conditions of patients with early renal damage in hypertension,the four-diagnosis information of traditional Chinese medicine,and clinical biochemical indicators,and use the statistical method of Logistics regression analysis to analyze the four types of patients with early renal damage in hypertension.Statistical analysis of diagnosis information,comparison of the correlation between different TCM syndromes and various clinical biochemical indicators,so as to provide a basis for guiding TCM dialectical treatment of early hypertension renal damage.MethodsCollect the clinical data of patients with primary hypertension who were in the first affiliated hospital of Guangzhou University of Traditional Chinese Medicine from June 2016 to June 2019.Combined with the relevant criteria of TCM syndromes,the medical record system is used to screen out the early kidneys.Patients who compromise the diagnostic criteria.Use the retrospective clinical information collection table to sort out relevant case data.The relevant contents are: general information,information on the four diagnosis of traditional Chinese medicine,clinical biochemical indicators(serum creatinine,blood uric acid,glomerular filtration rate,urine microalbumin,blood sugar,Total cholesterol,triglycerides,lowdensity lipoprotein,high-density lipoprotein,etc.),various TCM syndromes and their distribution.Analyze the collected data and TCM syndromes.ResultsHypertensive kidney damage is attributed to the categories of "vertigo","headache","edema",and "deficiency" in traditional medicine.Due to the complex etiology and the mutual influence of multiple pathogenesis,various schools have proposed a variety of treatment methods.A total of 190 patients were enrolled in this study.The main syndromes were liver-fire hyperactivity,yin deficiency and yang syndrome,phlegm-dampness stagnation syndrome,yin and yang deficiency syndrome,qi stagnation and blood stasis syndrome.There were 181 patients,accounting for the total number of patients.95.2%,other syndromes accounted for a relatively low proportion,a total of 9 patients,a total of about 4.8%.Among the five main syndromes,the patients with phlegm-dampness stagnation syndrome are the most,with78 cases,accounting for 43.1% of the total number;followed by the liverfire hyperactivity syndrome,with 70 cases,accounting for 38.1% of the total number;followed by Qi stagnation and blood stasis syndrome,There are 16 cases,accounting for 8.8% of the total number of people;there are11 cases of Yin deficiency and Yang hyperactivity syndrome,accounting for6.1% of the total number of people;the last is the syndrome of Yin and Yang deficiency,there are 6 cases,accounting for 3.3% of the total number.In general data,the oldest age is 89 years old,and the youngest age is 18 years old.Among them,there are 15 males in the youth group,accounting for 8.2%,2 females in the youth group,accounting for 1.1%;41males in the middle-aged group,accounting for 22.7 %,there are 38 women in the middle-aged group,accounting for 20.9%;32 men in the elderly group,accounting for 17.7%,and 53 women in the elderly group,accounting for29.4%.The proportion of female patients in the elderly group is the highest;the proportion of male patients in the young and elderly groups is lower than that of females in the same age group.Among the average ages,the average age of men is 58.63±12.37 years,and the average age of women is 65.88±16.84 years.Females are higher than men,and the difference is statistically significant(P<0.05).The average height of men was 170.18±4.96 cm,and the average height of women was158.63±5.20 cm.The height of men was higher than that of women,and the difference was statistically significant(P<0.05).Among the average BMI,the average BMI of men is 26.15±2.41 kg/m2,and the average BMI of women is 23.84±2.30 kg/m2.Males are higher than females,and the difference is statistically significant(P<0.05).In the regression analysis of TCM syndromes and related factors,the syndrome of phlegm-dampness is positively correlated with the history of smoking,negatively correlated with the history of hypertension,and negatively correlated with the history of drinking;the hyperactive liverfire syndrome is positively correlated with abnormal lipid metabolism,and smoking History is negatively correlated;Qi stagnation and blood stasis syndrome is negatively correlated with the patient’s hypertension;Yin deficiency and Yang hyperactivity syndrome is positively correlated with age.In the analysis and comparison of the biochemical indicators of patients with early renal damage in hypertension,the level of urine ACR has nothing to do with the gender of the included patients.Among the five syndromes,Yin deficiency and Yang hyperactivity syndrome and liver-fire hyperactivity syndrome are the most prominent for early renal damage.In liposugar metabolism,the overall difference of MALB between the different syndrome groups is not statistically different,and the overall difference of fasting blood glucose between the different syndrome groups is not statistically different,but LDL-C,triglycerides,and total cholesterol are not statistically different.The overall differences between the different syndrome groups are statistically different,and the liver-fire hyperactivity syndrome is positively correlated with abnormal lipid metabolism.Conclusion1.In general data,although the number of male patients is smaller than that of female patients,the overall onset is earlier,the history of smoking and drinking,and the phenomenon of overweight are more frequent.2.Patients with primary hypertension and early renal damage have the most patients with the syndrome of phlegm-dampness,and the least patients with the syndrome of yin and yang deficiency.3.Patients with hyper-hepatic fire syndrome should strengthen lipidlowering.The older the patient is,patients with early renal damage in hypertension are more likely to be dialectically diagnosed with Yin deficiency and Yang hyperactivity.There are the largest number of patients with phlegm-dampness syndrome,which may be a syndrome of prehypertension.The hypertension of patients with Qi stagnation and blood stasis syndrome is lower than that of patients with other symptoms of early renal damage of hypertension,and the risk of hypertensive crisis events is lower.4.The level of urinary ACR has nothing to do with the gender of the included patients.Yin deficiency and Yang hyperactivity syndrome and liver-fire hyperactivity syndrome are the most prominent in early glomerular damage.There was no statistical difference in overall glucose metabolism among different syndrome groups.There are significant statistical differences in the overall difference of lipid metabolism among different syndrome groups,and there is a positive correlation between liver-fire hyperactivity and abnormal lipid metabolism.5.Based on the above conclusions,for patients with primary hypertension and early renal damage,we can consider providing new ideas for "one person,one party" and "individualized treatment" of Chinese medicine based on TCM syndromes and biochemical
