Clinical Experience Of Professor Liu Min In Diagnosis And Treatment Of Gall Disease(Hashimoto’s Thyroiditis) Based On Data Mining

ObjectiveTo explore the medication rule and clinical thinking of Professor Liu Min in the treatment of gall disease(Hashimoto’s thyroiditis),and establish the medical record database of Professor Liu Min in the treatment of gall disease,so as to properly preserve his clinical experience and better inherit the academic thought of famous traditional Chinese medicine,and provide reference for more clinical workers in the use of traditional Chinese medicine in the treatment of gall disease and academic research in the future.MethodsA retrospective study was conducted to collect the medical records of gall disease treated by Professor Liu Min in the Department of Endocrinology,the First Affiliated Hospital of Guangzhou University of Chinese Medicine from January 1,2019 to December31,2019.Microsoft Excel software was used for frequency statistical analysis,SPSS modeler 18 software was used to construct the network diagram of high-frequency traditional Chinese medicine,Aprior modeling was used to complete association rule analysis,SPSS statistics 26 software was used to complete factor analysis and cluster analysis.result1.106 patients were included in this study,a total of 200 visits(200 cases of medical records),including 10 males and 96 females,ranging in age from 7 to 77 years old,of which 28 to 37 years old accounted for the largest proportion.2.According to the distribution of syndrome types,the 200 cases of medical records can be divided into Taiyang syndrome,Shaoyang syndrome,Yangming syndrome,Taiyin syndrome,Shaoyin syndrome,Jueyin syndrome and qi stagnation and phlegm coagulation syndrome.There are 33 cases of Taiyang syndrome.The common prescriptions are Guizhi Jia Longgu Muli Decoction,Guizhi Jia Fuzi Decoction and Guizhi Jia Huangqi Decoction.The common symptoms of Taiyang syndrome patients with gall disease are sweating,aversion to cold,evil wind,cough,reddish tongue,thin white moss,dry mouth,poor sleep,dreaminess,hair loss,etc.There were 108 cases of Shaoyang syndrome.The commonly used prescriptions were Chaihu Guizhi Decoction,Xiaochaihu Decoction,Xiaochaihu Decoction and Siwu Decoction.The common symptoms included dry mouth,bitter mouth,palpitation,chest tightness,irritability,sweating,poor sleep,dreaminess,easy to wake up,aversion to cold,aversion to heat,redness of tongue,pale redness of tongue,thin white fur,pulse string and so on.Only one case of Yangming syndrome was included in the 200 cases,and the prescription was Gegen Qinlian Decoction.There were 10 cases of Taiyin syndrome,and the commonly used prescriptions were Banxia Xiexin Decoction,Fuzi Lizhong pill,Ganjiang Lingzhu Decoction,etc.Common symptoms include abdominal distension,abdominal pain,abdominal cold,belching,rotten stool,red tongue,tooth marks on the edge of tongue,thin and white fur,etc.There were 33 cases of Shaoyin syndrome.The commonly used prescriptions were Sini Jia Renshen Decoction,Zhenwu Decoction and Fuzi Decoction.The main symptoms included light red tongue,dark red tongue,tooth marks on the edge of tongue,thin white fur,thin pulse,dry mouth,rotten stool,drowsiness,fatigue and poor sleep.Seven cases of Jueyin syndrome were commonly prescribed with Wumei Pill,Danggui Sini Decoction,Baitouweng Decoction,etc.The main symptoms were pale tongue,tooth marks on the edge of tongue,floating pulse,dry mouth,sweating,chest tightness,belching,rotten stool,incomplete feeling of stool,evil heat,abdominal cold,etc.There were 8 cases of qi stagnation and phlegm coagulation syndrome.The common prescriptions were Banxia Houpo Decoction,Banxia Houpo Decoction and Jiegeng Decoction,Banxia Houpo Decoction and Danggui Sini Decoction,etc.the common symptoms were cough,expectoration,dry mouth,foreign body sensation in pharynx,red tongue,fine pulse,etc.3.A total of 115 traditional Chinese medicines(TCMs)were included in 200 cases in this study,and the top 10 high-frequency TCMs were: Zhigancao,Fabanxia,Dazao,Guizhi,Huangqin,Chaihu,Chishao,Muli,Hongshen and Longgu.Most of the TCMs in Professor Liu Min’s prescriptions for gall treatment were warm,among the five flavours,sweet flavours were the most,and the number of flavours in the spleen meridian was the most.The classification of drug efficacy found that Professor Liu Min used the most Qi tonic traditional Chinese medicines.4.According to the association rules analysis of high-frequency traditional Chinese medicine,it is found that the commonly used drug pairs in Professor Liu Min’s prescription for treating gall disease include Longgu and Muli,Chaihu and Huangqin.The commonly used three drug combinations are Longgu-Muli-Zhigancao,Guizhi-Zhigancao-Dazao,Guizhi-Chishao-Zhigancao,etc.Factor analysis showed that five common factors could be extracted.The results of cluster analysis showed that when the distance between groups was 15,it could be divided into 8 groups.ConclusionIn the treatment of Hashimoto’s thyroiditis,Professor Liu Min used warm to expel cold,pungent to disperse,bitter to dry and wet,sweet to warm and tonic.Most of the drugs used belong to the spleen meridian,and the efficacy of drugs is mainly to replenish qi,which reflects that Professor Liu Min attaches importance to the position of spleen and stomach in the treatment of gall disease.In addition,in the treatment of gall disease,Professor Liu Min is good at using classical prescriptions treatment based on syndrome differentiation.Through the statistical analysis of the data,Professor Liu Min’s clinical experience was obtained,and the clinical treatment of Hashimoto’s thyroiditis in hyperthyroidism stage and hypothyroidism stage was explored,as well as the classic drug pairs commonly used in the treatment of Hashimoto’s thyroiditis.The general treatment principle of Hashimoto’s thyroiditis was summarized,which was " Far cold and dampness,warm spleen and kidney,smooth pivot mechanism ",and provided some experience for clinical workers in the treatment of Hashimoto’s thyroiditis in the future Reference and dialectical treatment.
