【Objective】To investigate the incidence of body dysmorphic symptoms(BDS)among inpatients in psychiatric departments of a general hospital and to explore the influence factors of BDS comorbidity.【Methods】Patients with major depressive disorder, anxiety disorder, bipolar disorder or somatic symptom disorder who were admitted to the Department of Psychosomatics and Psychiatry,Affiliated Zhong Da Hospital of Southeast University from November 2018 to January 2020 were enrolled. After obtaining the informed consent of the patients, the survey was conducted on the subjects who met the inclusion and exclusion criteria by using Self Rating Scale of Body Image(SRSBI),Patient Health Questionnaire-9(PHQ-9),Generalized Anxiety Disorder-7(GAD-7),Psychosomatic Symptom Scale(PSSS)and Eysenck Personality Questionnaire(EPQ).After two weeks of systematic hospitalization treatment in the Department of Psychosomatics and Psychiatry,a questionnaire survey was conducted again and all data were statistically analyzed.【Results】This study included 160 subjects,55 males(34.4%)and 105 females(65.6%),with an average age of(34.04±15.99)years.113 patients were diagnosed as major depressive disorder,15 patients as anxiety disorder,25 patients as bipolar disorder, and 7 patients as somatic symptom disorder.46 patients(11.9%)were diagnosed as positive for family history of mental disorders, including 11 patients(6.9%)of grade I,4 patients(2.5%)of grade II,and 4patients(2.5%)of grade III.The subjects were divided into BDS group and non-BDS group according to the baseline SRSBI score,of which 54 cases(33.8%)were in BDS group and106 cases(66.2%)were in non-BDS group. The patients of BDS group were significantly younger than non-BDS group patients, and the age was negatively correlated with SRSBI score. The scores of Psychoticism(P)factor and Neuroticism(N)factor in BDS group are higher than those in non-BDS group,in which P factor has a positive correlation with SRSBI score at baseline. The scores of item 1,15,16,17 and dimension x5 of SRSBI were significantly different among different disease groups at baseline, and the scores of item1,2,3,5,10,14,15,16,17 and dimension x1,x2,x4,x5,x6,x7 of SRSBI showed significant differences among different disease groups after treatment.Symptoms of all dimensions were siganificantly improved after treatment. The baseline scores of SRSBI,PHQ-9,GAD-7 and PSSS in BDS group are higher than those in non-BDS group. The baseline scores of PHQ-9,GAD-7 and PSSS are positively correlated with SRSBI scores. The SRSBI,PHQ-9,GAD-7and PSSS reduction rates in BDS group are lower than those in non-BDS group. The reduction rates of PHQ-9,GAD-7 and PSSS are negatively correlated with the SRSBI score at baseline and positively correlated with the SRSBI reduction rate.Logistic regression analysis showed that BDS was more likely to occur with younger age, higher P factor score, and higher baseline PSSS score.Multiple linear regression analysis was conducted on SRSBI baseline score. The results showed that patients with younger age and higher baseline PSSS score got higher SRSBI baseline score.【Conclusion】The prevalence rate of BDS in inpatients with major depressive disorder, anxiety disorder, bipolar disorder or somatic symptom disorder in psychiatric departments of a general hospital is 33.8%.Patients with BDS are characterized by young age, psychoticism personality and severe psychosomatic symptoms.BDS may exacerbate the depression, anxiety and psychosomatic symptoms in patients, and significantly affects the prognosis of patients.
