| Objective To investigate the effect of transcutaneous electrical acupoint stimulation on perioperative protein level in patients with colorectal cancer and to observe its effect on rapid recovery of patients with colorectal tumor after operation. Methods Sixty-five patients(25-75 years old,ASA grade Ⅰ-Ⅱ)undergoing laparoscopic radical colorectal cancer resection with general anesthesia were randomly divided into two groups: TEAS group(T group)and sham stimulation group(S group).Patients in group T were given TEAS for 30 minutes at Hegu(LI4),Neiguan(PC6),Zusanli(ST36),Shangjuxu(ST37),Xiajuxu(ST39)and Sanyinjiao(SP6)in one day before operation, and 30 minutes before anesthesia induction,at the beginning of skin incision and at the end of operation respectively.TEAS was given at the same acupoint on the 1st,2nd and 3rd day after operation, each time 30 min.The patients in group S were given sham stimulation at the same acupoint and time point as group T,and the changes of serum total protein(TP),albumin(ALB),prealbumin(PA)and transferrin(TRF)were observed and recorded on the first day before operation, the first day,the third day and the seventh day after operation. The changes of heart rate(HR)and mean arterial blood pressure(MAP)were recorded when patients entered the room(T0),after anesthesia induction(T1),after intubation(T2),during skin incision(T3),at the end of operation(T4)and after extubation(T5).And the first exhaust, defecation time, fluid diet time, appetite score at the beginning of eating and postoperative hospital stay were recorded. The incidence of postoperative nausea, vomiting, abdominal distension, incision infection, incision dehiscence and anastomotic leakage were recorded. Results There was no significant difference in the general data between two groups, but on the 1st and 3rd day after operation,TP,ALB,PA and TRF in both groups decreased significantly as compared with those on the first day before operation. On the 7th day after operation,TP,ALB,PA and TRF in group T gradually returned to the preoperative level(P>0.05),while those in group S were still lower than the basic value before operation and there was significant difference(P<0.05).On the 3rd and 7th day after operation,TP,ALB,PA and TRF in T group were significantly higher than those in S group.Compared with entering the room(T0),the heart rate(HR)of S group decreased more significantly after anesthesia induction(T1)(P<0.05);the heart rate(HR)decreased significantly at the time of skin incision(T3)and the end of operation(T4).The mean arterial pressure(MAP)decreased more significantly after anesthesia induction(T1)and at the end of operation(T4)than at the end of operation(T0),and the mean arterial pressure(MAP)in S group also decreased significantly at T3(P<0.05).The heart rate(HR)and mean arterial pressure(MAP)in group S were significantly higher than those in group T after intubation(T2)and extubation(T5).The first exhaust time, defecation time and liquid diet time in group T were significantly shorter than those in group S(P<0.05).There was no significant difference in appetite score and postoperative hospital stay between the two groups. The incidence of postoperative nausea, vomiting and abdominal distension in group T was significantly lower than that in group S(P<0.05),but there was no significant difference in the incidence of incision infection, incision dehiscence and anastomotic leakage between the two groups. Conclusion Transcutaneous electrical acupoint stimulation can improve the perioperative protein level, stabilize the perioperative hemodynamics, promote the recovery of gastrointestinal function and reduce the incidence of complications in patients with colorectal cancer. It has important clinical value for the rapid recovery of patients with colorectal cancer after operation. |
