Objective The purpose is to investigate the self-perceived burden level of urostomy patients and analyze its influencing factors.Based on constructing structural equation model,this paper discusses the mediating effect of resilience in the path of self-perceived burden on quality of life of patients with urostomy.The aim is to provide a data support for rising the quality of life in patients with urostomy from the perspective of self-perceived burden and resilience.Methods From April 1,2020 to October 1,2020,a total of 303 patients with urostomy in outpatient department of three tertiary hospitals in Yinchuan,Ningxia,were enrolled by convenience sampling method.A cross-sectional survey was conducted in the form of questionnaire,the survey tools included:general information questionnaire,Self-perceived Burden Scale(SPBS),City of Hope-Quality of Life-Ostomy Questionnaire(COH-QOL-OQ),Connor-Davidson Resilience Scale(CD-RISC),Social Support Rating Scale(SSRS).With the help of SPSS 25.0,using statistical methods to analyze the data to draw conclusions;with the help of Amos 21.0 software to build the structural equation model,using the Bootstrap BC program to test the mediating effect.Results1.The level of self-perceived burden in patients with urostomyThe average score of SPBS is 31.00±6.89.SPB was present among 89.8%patients,among them,168 cases(55.4%)had moderate SPB,followed by 74 cases(24.4%)with mild burden and 30 cases(9.9%)with severe burden.2.Influencing factors of self-perceived burden in patients with urostomyThe results of multiple linear stepwise regression analysis showed that degree of stoma self-care,complications,average monthly income,urostomy time and social support utilization entered the regression equation,F=41.133,R~2=0.409,P<0.001,which could comprehensively explain 40.9%variance of self-perceived burden regression equation.3.Quality of life in patients with urostomyThe total score of quality of life of patients with urostomy was 194.57±20.41,scale average score was 6.08±0.64.4.Resilience level in patients with urostomyThe resilience scale’s total score was 51.22±13.32 and the scale’s average score was2.05±0.53.5.There was a significant negative correlation between self-perceived burden and quality of life on the patients with urostomy,the self-perceived burden was negatively correlated with resilience,and the quality of life was positively correlated with resilience.6.The mediating role of resilienceThe results of path analysis and mediating effect test based on structural equation model shows that the direct negative effect of self-perceived burden on quality of life is significant,the direct effect value is-0.311,the indirect effect value is-0.530;the total effect value of self-perceived burden on quality of life is-0.841,and the mediating effect of resilience accounts for63.02%of the total effect.Conclusions1.Patients with urostomy generally have a burden of self-perceived feeling,and the overall burden is at a moderate level.The burden is mainly affected by the degree of stoma self-care,complications,average monthly income,stoma time,utilization of social support.2.The quality of life of patients with urostomy is low,the level of resilience is also low.3.There is a significant correlation among the self-perceived burden and the quality of life and resilience of patients with urostomy.4.Resilience plays a part of mediating regulation in the path of self-perceived burden on quality of life,and the degree of regulation reaches 63.02%.The self-perceived burden of patients with urostomy will ultimately affect their quality of life,and resilience plays a mediating role in it.Therefore,nurses could strengthen continuing care and improve social support.Personalized psychological intervention measures can be taken to help patients with cognitive and emotional regulation.Reducing the self-perceived burden of patients with urostomy by improving the level of psychological resilience,could rise the level of quality of life...
