| Objective:To evaluate the safety and efficacy of percutaneous nephrolithotomy(PCNL)in oblique supine position and prone position in the treatment of renal or upper ureteral calculi.Methods:Search Pub Med,Web of Science,The Cochrane Library,Embase,CNKI and other related literature database,according to the inclusion and exclusion criteria to screen the clinical control study on oblique supine position and prone position PCNL,extract the data included in the study about stone clearance rate,operation time,hospital stay and the incidence of perioperative complications and Meta analysis.Results:A total of 8 references were included and the total number of cases was765.Meta-analysis showed that there was no significant difference in stone-free rate between the oblique supine position group and the prone position group [OR=0.77,95%CI(0.49,1.21),P=0.26],and the operation time in the oblique supine position group was shorter than that in the prone position group [MD=-17.26,95%CI(-29.98,-4.54),P=0.008].There was no significant difference in hospitalization time between oblique supine position group and prone position group[MD=-0.17,95%CI(-0.81,0.47),P=0.60],and there was no significant difference in the incidence of perioperative complications between oblique supine position group and prone position group [OR=0.85,95%CI(0.59,1.23),P=0.39].In oblique supine group and prone group,fever[OR=0.92,95%CI(0.52,1.26),P=0.79],hemorrhage [MD=0.02,95%CI(-0.20,0.24),P=0.85],blood transfusion[OR=0.66,95%CI(0.20,2.15),P=0.49],urine leakage [OR=0.78,95%CI(0.29,2.14),P=0.64],intestinal injury [OR=0.20,95%CI(0.01,4.18),P=0.30] There was no significant difference between them.Conclusions:The stone-free rate of PCNL in oblique supine position is similar to that in prone position,and PCNL in oblique supine position can shorten the operation time,so it is a safe and effective choice for the treatment of renal or upper ureteral calculi. |
