| Objective: To investigate the changes of serum iridin level in patients with acute coronary syndrome and its relationship with the degree of coronary artery stenosis.Methods:1.A total of 127 patients who underwent coronary angiography in the second Hospital of Hebei Medical University from February 2020 to December 2020 were divided into control group and acute coronary syndrome group according to the results of coronary angiography and symptoms,ACS group was divided into three subgroups according to ECG and myocardial enzymes : ST segment elevation myocardial infarction group,non-ST segment elevation myocardial infarction group and unstable angina pectoris group.2.According to the results of coronary angiography,SYNTAX score was performed and divided into three groups: low risk group(≤ 22 points),medium risk group(23-32 points)and high risk group(≥ 33 points).3.The general clinical data and biochemical indexes of the subjects were collected,and the serum Irisin level was detected by ELISA method,and the data were processed by SPSS22.0 software.Results:1.Comparison of general clinical data between CON group and ACS group(UA group,NSTEMI group and STEMI group): there was no significant difference in age,BMI,creatinine,total cholesterol,triglyceride,high density lipoprotein cholesterol,low density lipoprotein cholesterol,history of hypertension and smoking.There was significant difference in left ventricular ejection fraction(P <0.05),but there was no significant difference between the other two groups(P > 0.05).2.Comparison of Irisin levels among groups: the average value of Irisin in the control group was significantly higher than that in the ACS group(282.06±106.51pg/m L vs 100.07±36.83pg/m L,P<0.05).The average value in control group(282.06 ±106.51pg/m L)was significantly higher than that in UA group,NSTEMI group and STEMI group(121.32±39.15pg/m L,105.09±40.00pg/m L,82.11±21.68pg/m L,P < 0.05).The average value in UA group was significantly higher than that in STEMI group(121.32±39.15pg/m L vs 82.11±21.68pg/m L,P<0.05).There was no significant difference between the other two groups.3.Correlation analysis between serum Irisin and other indexes: there was a significant positive correlation between serum Irisin level and LVEF(r=0.363,P<0.001),and the difference was statistically significant.There was a significant negative correlation between serum Irisin level and SYNTAX score(r=-0.411,P<0.001),and the difference was statistically significant4.Correlation analysis between Irisin level and the degree of coronary artery stenosis in ACS patients: the mean of low risk(110.41±33.44pg/m L)was significantly higher than that of median and high risk(94.69±36.25 pg /m L,73.61±37.07 pg / m L,P < 0.05).The differences were statistically significant.5.Analysis of influencing factors of coronary artery disease: SYNTAX score was taken as dependent variable,sex,age,Irisin,BMI,CR,total cholesterol,triglyceride,HDL,LDL,LVEF,history of hypertension and smoking were used as independent variables.The results showed that there was a linear regression relationship between Irisin and SYNTAX,and Irisin was an independent influencing factor independent of sex,age,BMI and other indexes.Conclusion:The level of serum Irisin in patients with ACS is decreased,and it is negatively correlated with SYNTAX score,which may inhibit the occurrence and development of ACS through related ways,and has the potential to become a biomolecule to assist in the diagnosis of ACS. |
