Objective: The purpose of this study is to investigate the awareness of the use of benzodiazepines among psychiatrists in Hebei Province,to analyze their mastery of pharmacological mechanism and drug use principles,to understand their drug preference,combined use and cognition of drug side effects,and to analyze the related influencing factors,so as to improve the clinical practice and supervision more scientifically and reasonably,comprehensively improve the rational use of BZDs by psychiatrists,and effectively prevent the occurrence of BZDs abuse and addiction.Methods: This study conducted a questionnaire survey of psychiatric departments in 50 psychiatric hospitals or general hospitals in Hebei Province(Baoding,Cangzhou,Hengshui,Chengde,Handan,Langfang,Qinhuangdao,Shijiazhuang,Tangshan,Zhangjiakou),The questionnaire was designed by the Institute of Mental Health of Hebei Medical University and approved by the Ethics Committee of the First Hospital of Hebei Medical University.The content is divided into two parts,the first part: the general situation of the doctor,including city,gender,age,education level,working years,education,professional title,and the level of the hospital;The second part: BZDs related knowledge,including four modules,module one is "mechanism of BZDs",module two is "classification and use principles of BZDs",module three is "combined application of BZDs",module four is "side effects of BZDs".Modules 1 and 2 are objective questions to judge the psychiatrist’s awareness of the mechanism and classification principles of BZDs.The specific method is 50 points for each module,totaling 100 points,and 4 levels of awareness are analyzed.Excellent scores are greater than 80 points,good scores range from70 to 79 points,qualified scores range from 60 to 69 points,and unqualified scores are less than 60 points.Modules 3 and 4 investigate doctors’ knowledge of medication preferences,combination medications,and side effects.Results: A total of 628 psychiatrists were investigated.572 questionnaires were collected,accounting for 91.1%,of which 505 were valid,accounting for 88.3%.Among the sevalid samples,there were 199 males(39.4%)and 306 females(60.6%).Among these505 doctors,there were 122residents(24.1%),214 attending physicians(42.4%),116 associate chief physicians(23.0%)and 53 chief physicians(10.5%).106 doctors(21.0%)came from tertiary hospitals,319 doctors(63.2%)came from secondary hospitals,and 80 doctors(15.8%)came from other hospitals.According to the awareness rate of doctors,excellent score was more than 80 points,accounting for 5.0%(25/505),good score was 70~79 points,accounting for 34.2%(173 /505),qualified score was 60~69 points,accounting for 44.0%(222 / 505)and unqualified score was 16.8%(85 / 505);The total qualified rate of BZDs used by psychiatrists in Hebei Province was 83.2%.The one-way analysis of variance of the psychiatrist’s total awareness score of benzodiazepine drugs showed that working years had a significant impact on the results(P=0.031).In the treatment of patients with depression,the combined application time of antidepressants and BZDs is 2-4 weeks,accounting for 42.38%.When dealing with manic patients,BZDs used "often" accounting for 66.93%.When dealing with patients with schizophrenia,55.25% of them choose “often” in combination with BZDs.Conclusion: The overall awareness of benzodiazepine drug use by psychiatrists in the psychiatric hospital or general hospital of Hebei Province is relatively good,but the excellent rate is low.The overall knowledge of psychiatrists on the mechanism of action and principles of use of BZDs is mainly related to the length of work.As the length of work increases,the awareness score gradually increases.In the treatment of patients with depression,the most commonly used benzodiazepine is Lorazepam,and the least commonly used is Midazolam.When treating manic patients,psychiatrists are more inclined to use BZDs frequently.When treating patients with schizophrenia,doctors with high academic qualifications,professional titles,and hospital levels are more inclined to sometimes use BZDs in combination.
