The Value Of SOFA Score,RDW,PDW In Prognosis Evaluation Of Sepsis

Objective: To investigate the value of SOFA score,RDW,PDW in prognosis evaluation of sepsis.Methods: Patients admitted to the Department of Intensive Care Medicine of the Union Hospital of Fujian Medical University from January 1,2019 to June 30,2020,with the diagnosis of infection or suspected infection,and evaluate the first 24-hour sequential organ failure score(SOFA score),Sepsis was diagnosed with SOFA score above 2,Patients with septic shock can be identified with a clinical construct of sepsis with persisting hypotension requiring vasopressors to maintain MAP≥65 mm Hg and having a serum lactate level>2 mmol/L despite adequate volume resuscitation.Clinical data were collected,which mainly included blood routine examination,biochemical test,lactate,procalcitonin,blood gas analysis,bacterial culture and the worst SOFA score,hospitalization days,28-day and mortality.The patients were divided into survival group and non-survival group according to 28-day mortality.The risk factors of death in sepsis patients were analyzed by multivariate logistic regression,and the ROC curve was drawn to calculate the area under the curve and compare the predictive value of each factor on the death of patients with sepsis.Results:(1)A total of 160 patients were enrolled,112 patients in the survival group,and 48 in the non-survival group.(2)The age,gender,SOFA score,bacterial culture positive rate,blood culture positive rate,WBC,N%,RDW-SD,RDW-CV,PDW,TBIL,UREA,CR,PCT in the non-survival were higher than those in the survival group,and the length of hospital stay was shorter than that in the survival group.The difference was statistically significant(P<0.05).AND the ICU hospitalization,sepsis or sepstic shock,source of infection,comorbidities,RBC,HB,PLT,ALB,ALT,AST,r-GT,ALP were no significant differences between the two groups(P>0.05).(3)The logistic regression analysis showed that the SOFA score,RDW-CV and PDW are independent risk factors for the prognosis of patients with sepsis.(4)In terms of prognosis,The AUC of SOFA score was 0.743,the AUC of RDW-CV was 0.703,and the AUC of PDW was0.765.Conclusions: SOFA score,RDW-CV and PDW are independent risk factors for the prognosis of patients with sepsis,and the effective of them is equal.
