Objective: To analysis the clinical characteristics of parathyroid cysts and parathyroid adenomas,explore the possible predictive factors,and reduce missed diagnosis and misdiagnosis.Methods:1.153 patients with parathyroid diseases(including 56 cases of parathyroid cyst and 102 cases of parathyroid adenoma)who underwent surgery in the second Hospital of Hebei Medical University from March 2013 to December 2020 were collected.The clinical data were analyzed retrospectively,and the differences in clinical manifestation,biochemistry,imaging and pathology between parathyroid adenoma and parathyroid cyst were compared.2.SPSS23.0 was used for statistical analysis,P<0.05 was considered statistically significant.Results:1.In this study,there were 56 patients with parathyroid cyst and 102 patients with parathyroid adenoma.There were significant differences in diagnosis age,preoperative biochemical index,tumor diameter and volume.2.The diameter of parathyroid cyst was related to sex,while the diameter of parathyroid adenoma was related to serum calcium level,and the higher the parathyroid hormone before operation,the larger the volume of parathyroid adenoma.3.Logistic regression analysis suggested that corrected calcium concentration and serum phosphorus level could be used as important biochemical indexes to distinguish parathyroid adenoma from non-functional parathyroid cyst.Conclusions:1.Both parathyroid adenomas and parathyroid cysts were more common in middle-aged postmenopausal women.Patients with parathyroid cysts were relatively young,and the volume of parathyroid cysts was usually larger than that of adenomas.Parathyroid cysts were single cysts,while there can be multiple adenomas.Bilateral exploration during operation was recommended when considering parathyroid adenomas without complications as far as possible.2.The concentration of corrected calcium and the level of serum phosphorus could be used as important biochemical indexes to distinguish the two kinds of diseases.3.Most patients with parathyroid adenoma and parathyroid cyst had no obvious clinical symptoms,but parathyroid cysts were given priority in the case of normal blood calcium.Patients with parathyroid adenoma could also show hyperparathyroidism with normal blood calcium.4.Both parathyroid cysts and parathyroid adenomas might be complicated with thyroid lesions,so it was necessary to comprehensively and accurately evaluate the thyroid gland before doing operation,and pay attention to the exploration of thyroid lesions during the operation.If malignant thyroid resection and lymph node dissection could be performed at the same time to improve the prognosis of the patients.
