Background:Specific increase in slow wave activity and/or decrease in fast wave activity in electroencephalography during acute ischemic stroke(AIS).Quantitative electroencephalography(q EEG)is highly sensitive to abnormal brain electrical activity and may have its unique application value in AIS.Objective: To explore the application value of quantitative EEG index in the auxiliary diagnosis of AIS and prediction of functional prognosis.Methods:68 patients with acute ischemic stroke and 66 healthy controls were included.Demographic information(age,gender,etc.)and previous medical history were collected,and the degree of clinical functional deficit and imaging characteristics of the AIS group were collected.The EEG of the healthy control group and the AIS patients within 72 hours of the onset of the disease were collected,and five EEG indexes(DTABR,DAR,RDP,RAP,pd BSI)were analyzed for the ability to diagnose AIS.According to MRS score,MRS ≤2 was divided into good prognosis group,and MRS ≥3 was divided into poor prognosis group.After adjusting for age and NHISS,the ability of these 5 EEG indexes to predict poor prognosis was analyzed.Results:Gender,age,hypertension,diabetes mellitus,hyperlipidemia and smoking history were not the influencing factors in AIS group and control group(P > 0.05).In binary Logsitic regression analysis,all the five QEEG indices could identify stroke(P < 0.05),and DTABR(AUC=0.89)and DAR(AUC=0.85)were the indicators with high discrimination ability.Age and NHISS were the influential factors for poor prognosis(P < 0.05).After adjustinginfluencing factors of age and NHISS,DAR,RDP,DTABR and RAP could predict the prognosis of poor function(P < 0.05).The indicators of high discrimination ability were DAR(AUC=0.92)and RDP(AUC=0.91).Conclusion:1.EEG index has clinical value in the early diagnosis of AIS,among which DTABR and DAR are more accurate in AIS recognition;2.This study verified that age and NHISS are indicators to predict the severity of prognosis of stroke;3.After adjusting for age and severity of clinical symptoms,EEG index was further found to predict functional outcomes after3 months of acute ischemic stroke,with higher accuracy in DAR and RDP.It provides a relative quantitative index for judging the prognosis in clinical work.
