Objective To systematically review the efficacy and safety of insulin combined with low molecular weight heparin in the treatment of hyperlipidemic acute pancreatitis.Methods Pub Med,Cochrane Library,Embase,Web of Science,CNKI,Wanfang Data,VIP and CBM were searched for related articles published up to November 2020.Two reviewers independently performed article screening,data extraction,and quality assessment,and meta-analysis was conducted using Rev Man 5.3.Results A total of 9 randomized controlled trials were included,with727 patients in total.Comparing with the combined treatment group,it is statistical significance(P < 0.05)of the significant efficiency rate,the total effective rate,gastrointestinal function recovery time,triglyceride level,CRP level,IL-6 level and TNF-α level in LMWH treatment group;It is statistical significance(P < 0.05)of gastrointestinal function recovery time,hospitalization time,triglyceride level and CRP level,but not of IL-6level in insulin treatment group;It is statistical significance(P < 0.05)of gastrointestinal function recovery time,triglyceride level and CRP level in conventional treatment group.The incidence of adverse drug reactions(ADR)was low in both groups,and there was no significant difference.Conclusion Compared with conventional therapy or monotherapy,insulin combined with low molecular weight heparin has significant efficacy and better safety in the treatment of hyperlipidemic acute pancreatitis,and may be considered as a first-line treatment.
