ancient books research.Literature review:Using traditional literature research methods to collect ancient and modern related literature,and use "menstrual disease blood syndrome" as a summary of the name of the gynecological blood syndrome related to menstrual disease.The research content of this article mainly includes the five diseases of premenstrual period,menorrhagia,prolonged menstrual period,bleeding between menstrual periods,and metrorrhagia,discuss its clinical characteristics and the etiology and pathogenesis of "deficiency,heat and blood stasis".In chronological order,trace the ancient doctors’ understanding of menstrual blood syndrome and the prescriptions for treatment,focusing on the review of the common syndrome differentiation and treatment of menstrual diseases by modern physicians,the syndrome differentiation is based on the types of viscera,qi,blood,yin and yang,and etiology and pathogenesis,treat the symptoms according to the principle of "emergency cures the symptoms,slow cures the root cause" Modern doctors have different treatment methods.They either emphasize the method of invigorating the kidney,or treating the liver as a reliever,or they create their own eight methods,etc.Finally,it discusses the progress of modern clinical diagnosis and treatment of menstrual blood syndrome from traditional dialectical treatment,adjustment of artificial cycle treatment,and other comprehensive treatments(including acupuncture,abdominal acupuncture,acupoint application,etc).Research on Ancient Books:Purpose: This research topic uses the fifth edition of the Chinese Medical Dictionary as the main search tool to retrieve all relevant ancient literatures that meet the TCM diagnosis of menstrual blood syndrome from the Qin and Han Dynasties to the Ming and Qing Dynasties and screen the prescriptions and medicines,then use the Chinese medicine inheritance auxiliary platform software(V3.0)independently developed by Yang Hongjun and others from the Institute of Chinese Materia Medica,China Academy of Chinese Medical Sciences,to perform data mining on the screening prescriptions,analyze and summarize the medicine experience and prescription rules of ancient doctors in the treatment of menstrual blood syndrome,and provide a certain theoretical basis for the clinical treatment of such diseases.Method: Enter the search terms summarized in the search system of the fifth edition software of "Chinese Medical Dictionary" and search in the "text" one by one,then screen out the relevant ancient books medical records for the treatment of menstrual blood syndrome that meet the inclusion and exclusion criteria,enter the relevant ancient books information collection table in the Excel software,and establish a database of ancient books and prescriptions,and import the database into the traditional Chinese medicine inheritance auxiliary platform software(V3.0)for data mining,and use the analysis functions of association rule analysis,complex system entropy hierarchical clustering and other analysis functions attached to the software to analyze the rules of composition in the data.Try to find new ways on the basis of this.Result: 1.There are a total of 580 prescriptions in ancient books related to the treatment of menstrual blood syndrome,involving a total of 430 drugs,of which 66 drugs are used more than 20 times,accounting for 77.8% of the total frequency of drug use.The top ten of all drugs in terms of efficacy are tonic,clearing away heat,warming,promoting blood circulation and removing blood stasis,hemostasis,relieving the surface,regulating qi,diuresis and dampness,collecting astringent,and dispelling rheumatism;The return meridian is in the order of liver,spleen,heart,kidney,lung,and stomach meridian;the order of the four qi is warm,cold,calm,hot,and cool;the order of the five flavors is sweet,bitter,pungent,sour,and salty.2.In this study,the support was set at 58,and 65 core medicine groups were obtained,involving 19 Chinese medicines,which are basically the same as the top 20high-frequency medicines.In this research,the k-means clustering algorithm is used and the k value is set to 4.That is,the ancient medical record data of menstrual blood syndrome is divided into 4 categories,and unsupervised clustering is performed,and 4 core prescription combinations appear.In conclusion: The treatment of menstrual blood syndrome in ancient medical records is mainly based on nourishing blood and nourishing qi drugs,followed by drugs for clearing heat and cooling blood,hemostasis and promoting blood circulation,corresponding to the etiology and pathogenesis characteristics of menstrual blood syndrome "deficiency,heat,and stasis".According to clustering the new prescription summaries obtained from the analysis can learn from the prescriptions and inherit the ancient doctors’ thoughts on the treatment of menstrual diseases and blood syndromes,and can also use this as a basis to further study the mechanism of its treatment,and provide more practical and effective ancient literature data support for the clinic.Clinical research:Purpose: This study sorted out the medical records of patients with menstrual blood syndrome treated by the supervisor,Chief Physician Wang Jianling,and analyzed,summarized and summarized the prescriptions in the cases through data mining with the aid of the "Traditional Chinese Medicine Inheritance Assistant Platform(V3.0)" system,So as to summarize the rule and prescription characteristics of Wang’s use of traditional Chinese medicine prescriptions for the treatment of menstrual blood syndrome,and compare and analyze with the research data of ancient books and literature to study the correlation between the two,so as to achieve the purpose of discovering new drug combinations and new prescriptions,and providing literature evidence and clinical reference value for modern Chinese medicine treatment of hemorrhagic menstrual diseases.Method: This study collected cases from patients who were treated by chief physician Wang Jianling from September 2019 to January 2021 and were treated with decoctions.According to the criteria for admission,487 cases were screened out(including 303 cases of metrorrhagia and prolonged menstrual period),79 cases,61 cases of premenstrual period,41 cases of menorrhagia,1 case of intermenstrual bleeding).After checking the clinical case information,enter it into the Excel table to form the original database,and use the traditional Chinese medicine inheritance auxiliary platform software(V3.0)to perform association rules and cluster analysis on the imported case data to chief physician Wang Jianling Data mining and development of new prescriptions are carried out on physicians’ clinical medication rules,and their similarities and differences are compared with the results of the previous study.Result: 1.A total of 485 prescriptions were entered into the TCM inheritance auxiliary platform for statistical analysis.The average age of the patients was 33.69±13.29 years old,and the highest proportion of the population was sexually mature(18-48 years old)women;the frequency of medication was greater than or equal to 40 times,a total of 60 prescriptions Traditional Chinese medicine accounts for 81.85% of the frequency of all medicines;the order of the four qi and five flavors is the same as that of ancient books,with the highest proportions being warm medicine and plain medicine;the order of the meridian distribution of all medicines is liver,spleen,kidney,lung,heart meridian;The highest proportion of its drug efficacy ranking is also the tonic drugs,followed by the heat-clearing,hemostasis,qi-regulating,hydration and dampness,activating blood and removing blood stasis,relieving the surface,resolving phlegm,relieving cough and relieving asthma,and reducing astringency.Class and Wenli class.Get 17 commonly used drug pairs and combinations,including 9traditional Chinese medicines,which are basically the same as high-frequency drugs.After setting the degree of correlation and punishment,the drug rule analysis of the above-mentioned drug pair combinations can obtain 26 core combinations of 2 to 5 drugs,and5 new prescription combinations can be obtained.2.Analyzing the rule of prescription,the degree of support was 145,and 76 core medicine groups were obtained,including 41 right medicine,27 corner medicines and 8 multi-flavored medicine groups,involving a total of 14 traditional Chinese medicines,which are basically in line with the top 15 high-frequency medicines.Also set the k value to 4,through the k-means algorithm,combined with teacher experience,you can also get 4 core prescription combinations.In conclusion:Data mining was conducted on the medication rule of the supervisor,Chief Physician Wang Jianling in the treatment of menstrual blood syndrome,it is concluded that Wang’s treatment is mainly based on drugs for warming the kidney and strengthening the spleen,nourishing blood and stopping bleeding,followed by clearing away heat and cooling blood,regulating qi to dissipate dampness,clustering new prescriptions shows that Wang’s treatment focuses on replenishing the spleen and treating blood with qi,which provides new treatment ideas and reference value for the clinical treatment of menstrual blood syndrome.Correlation comparison:Based on the data mining research of the first two parts,comparing the medication rules of ancient physicians and Wang in the treatment of menstrual blood syndrome,it can be seen that although the specific treatment medications are different,the medication ideas and principles of the two are similar,and both are supplementary.Deficiency,clearing away heat,removing blood stasis,and regulating qi are the mainstays.Deficiency drugs accounted for the most important medications.However,ancient physicians used drugs to treat the heart meridian,while Wang used lung meridian drugs as a supplement.In terms of formula compatibility,both of them focused on regulating qi,blood,yin and yang.However,ancient physicians emphasized "regulating menstruation from blood",and Wang combined the three methods of"blocking flow,clearing the source,and rejuvenating" to stop bleeding and "tonifying the kidney,strengthening the spleen,the idea of "Shugan" regulating menstruation aims to restore or establish a normal menstrual cycle.From this,it can be seen that Wang’s treatment of menstrual bleeding syndrome has not only learned and inherited the wisdom of the sages,but also based on this,she has been flexibly connected,and combined with modern medicine and clinical practice,he has added his own experience,dialectical medicine,and formed a unique treatment.Thought,so the clinical effect is very significant,it is worth learning and thinking in clinical practice. |
