Purpose:This study took 36 children with infantile spasms as the research object,using retrospective research methods.First,analyze and summarize their clinical characteristics.Second,summarize and refine the rules of the tutor Professor Ma Rong’s use of traditional Chinese medicine to treat the disease.Methods:Collected and selected cases of infantile spasms(36 cases in total)who were in the Ma Rong Pediatric Clinic of the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine in the past ten years from January 1,2011 to December 31,2020.Through statistics,basic information about the children’s age of onset,onset form,frequency,personal history,past history,family history,auxiliary examinations,medication use,prognosis,etc.;according to the efficacy evaluation system,they are divided into effective and ineffective groups,use SPSS23.0 software to perform data statistics and classify the data in the two sets of data.Among the measurement data,the t-test is used for the measurement data that meets the normal distribution and the variances are uniform;the rank sum test is used for the non-conformance;the count data is passed through the card Square test,Spearman rank correlation analysis,single independent variable or multi-independent logistic regression and other methods are used for testing.When p<0.05,the difference between the two groups of data is considered to be statistically significant.and the two groups are compared.Find out the influencing factors;collect information on the use of drugs and use the traditional Chinese medicine inheritance auxiliary platform(V2.5)to analyze the types of prescriptions,four qi,five flavors,and return to the meridian,and summarize the academic thoughts of Professor Ma Rong on the treatment of infantile spasms based on syndrome differentiation.Result:(1)The clinical characteristics of 36 children with infantile spasms:(1)Basic information of the cases: The male to female ratio in 36 cases was 2.27:1,and there were more boys.The overall age of onset was concentrated in 4-6 months after birth.Maternal education is mainly based on bachelor’s degree,and the age of first diagnosis is concentrated between 13 months and 18 months after birth;the length of consultation is concentrated between 0 and 6 months;(2)The cause of onset,changes in symptoms,and development: the main cause of illness is injection of vaccines As the children grow older,50% of the children will have 2 seizure changes(mainly clonic seizures,tonic seizures and absence seizures);except for one case where the relevant growth and development conditions are not recorded,the remaining 35 All the children had different degrees of language and motor development delay,the probability was 100%;(3)Medical history data: The average number of pregnancy of the mother of the child was 1.83,the average number of births was 1.47;the full-term children accounted for86.1%,and the cesarean section The children accounted for 55.6%;the most frequent abnormalities in the past history were neonatal mental retardation(56.3%)and neonatal ischemic hypoxic encephalopathy(43.8%);family history was mainly history of epilepsy(60%),The history of allergy is mainly drug allergy(77.8%);(4)Auxiliary examination: The abnormal rate of EEG in children was 100%,among which 47.2% was high arrhythmia;the total abnormal rate of head CT/MRI was 82%,among which the abnormal rate of effective group was 69%,and the abnormal rate of invalid group was 94%;4 The CDKL5 gene mutation rate was the highest among children with gene mutations(50%);(5)Western medical treatment: The most AEDs taken by children were VPA(61.1%)and TPM(55.6%);the overall average number of AEDs used was 2.06;The use rate of ACTH shock therapy was63.9%,and the effective rate was 55.6%;the use rate of VGB was 47.2%,and the effective rate was 25.0%;the use rate of KD was 30.6%,and the effective rate was 30.0%;another child had undergone surgery.The condition relapsed later.(6)Relationship between treatment time and curative effect: the best curative effect was 6 months after treatment,and the effective rate was 53.8%.According to statistical analysis,the difference between treatment time and curative effect is statistically significant p<0.05;children’s gender,parental and maternal education,age of onset,symptoms after change,number of pregnancies,high EEG irregularities,number of AEDs taken,etc.The difference between the data and the efficacy was not statistically significant p>0.05.(2)Research on the law of TCM syndromes and treatment:(1)The frequency of prescription of Chinese medicine: 36 children used prescription frequency of 248 times,effective group prescription frequency was 104 times,invalid group was 144 times;effective group average prescription frequency per person was 5.47 times,The ineffective group was8.47 times;after self-control,the frequency of effective prescriptions with symptoms improved by 50% or more was 96 times,including 77 effective decoctions and 19 effective Chinese patent medicines;5 most commonly used effective prescriptions(Ditan Decoction,Ditan Decoction,Baihe Decoction,Chaigui Longmu Decoction,Yinqiao Powder,Guzhen Decoction),2 kinds of effective Chinese patent medicines(Xifeng Capsules,Rongchang Capsules);(2)Characteristics of effective prescriptions: commonly used effective prescriptions include warming and calming for the four qi,pungent and sweet for the five flavors,and entering the lungs and spleen meridian as the main meridian.The four qi of Ditan Decoction is mainly warm and calm,the five flavors are mainly pungent,sweet and bitter,and the meridians are mainly entered into the spleen and liver;the four qi of Baihe Decoction are mainly warm,calm and cold,and the five flavors It is mainly sweet,bitter,and pungent,the meridian is mainly used to enter the lung and the spleen;the four qi of Chaiguilongmu decoction is mainly warm,cold and calm,and the five flavors are mainly pungent,bitter,and sweet.The spleen and lungs are the main meridians;the four qi of Yinqiao powder is mainly cold and flat,the five flavors are mainly bitter,pungent,and sweet,and the meridian is mainly used to enter the lung and the stomach;the four qi of Guzhen Decoction is mainly Mainly calm and warm,the five flavors are mainly sweet and pungent,and the main meridians are to enter the spleen and lungs.Conclusion:Children with infantile spasms are more male than female,and the age of onset is mostly4-6 months after birth;50% of children have 2 seizure changes,often changing to clonic seizures,tonic seizures and absent seizures;the main cause of the disease Vaccination,the most common neonatal ischemic hypoxic encephalopathy in the past,all children have different degrees of psychomotor developmental delay and abnormal EEG;when the treatment is 6 months,the patient suffers Children have the best curative effect,so the best course of treatment for infantile spasms with Chinese medicine is at least 6 months.The tutor’s prescriptions mainly consist of 5 types(Ditan Decoction,Baihe Decoction,Chaigui Longmu Decoction,Yinqiao Powder,Guzhen Decoction),corresponding to the treatment of five syndrome types(phlegm-qi block orifice syndrome,Shaoyang liver wind syndrome,external wind induced syndrome,There are mainly two kinds of effective Chinese patent medicines(Xifeng Capsules,Rongchang Capsules)for Yin deficiency and wind movement syndrome and Yang deficiency and slow convulsion syndrome.
