[Objective]To investigate the effect of iodine-131 treatment on cervical lymph node ultrasound features in patients with papillary thyroid carcinoma who had thyroidectomy.[Methods]Retrospective analysis of cervical lymph node ultrasound features of whom suffered thyroidectomy with papillary thyroid carcinoma.The patients(treatment group,133 cases,control group,89cases)who followed-up in our hospital from February 2018 to January 2019 were split into two groups which are based on iodine 131 treatment or not.The observed objects was cervical lymph nodes of ultrasound images,the transverse and longitudinal analysis of lymph node ratio,boundary,morphology,cortex echo,clumps strong echo in the cortical,hilus,microcalcification and cystic degeneration,the distribution of blood flow.Statistics of the number of cases in benign signs and malignant signs between the treatment group and control group.Chi-square test/Fisher exact test was used for each ultrasound sign(P<0.05 was considered statistically significant)to analyze whether the differences in the number of benign signs and suspected malignant signs between the treated group and the untreated group were statistically significant.Binary logistic regression analysis was performed on the ultrasound signs with statistical difference between the treated group and the untreated group.[Results]The proportion of cortical hyperecho and abnormal blood flow in cervical ultrasound of patients with papillary thyroid carcinoma after iodine-131 treatment was significantly higher than that of patients without iodine-131 treatment,and the difference was statistically significant(P =0.000,0.023,respectively).There were no significant differences in the ratio of transverse to longitudinal lymph nodes,margin,form,cortical mass hyperecho,hilum and calcification between the treated group and the untreated group(P = 0.262,1.000,1.000,0.329,0.081,0.321,respectively).There were no cases of cystic degeneration lymph node between the treated group and the untreated group.Binary logistic regression analysis showed that only iodine-131 treatment had a statistically significant effect on cortical hyperecho(P = 0.000),with OR value of 3.871.Gender and age had no statistically significant effects on cortical lymph node echo enhancement(P = 0.304 and 0.092,respectively).Iodine-131 treatment,age and gender had no statistical significance on lymph node blood flow distribution(P= 0.997,0.957,0.087,respectively).[Conclusion]Iodine-131 treatment has an effect on cortical echo of cervical lymph nodes in patients with papillary thyroid carcinoma,may enhance the echo of the cervical cortical lymph nodes.Thus,lymph node hyperecho should be more carefully to estimate benign or malignant for the patients of thyroid papillary carcinoma patients with iodine 131 treatment.Other auxiliary ultrasound examination and serological indexes are helpful to diagnosis suspicious malignant lymph node to avoiding misdiagnosis and unnecessary traumatic follow-up examination and treatment.
