purpose:Diabetic nephropathy(DKD)is a complication of diabetes(DM)and is related to abnormal metabolism.Hyperuricemia(HUA)as a metabolic disease is an independent risk factor for the development of chronic kidney disease.Diabetic nephropathy combined with hyperuricemia The patient has two abnormal metabolic factors at the same time.By discussing the distribution of TCM syndromes in patients with diabetic nephropathy and hyperuricemia and its correlation with clinical indicators,looking for the effects of diabetes and hyperuricemia on the kidneys when two unfavorable factors exist simultaneously.Provide research data to delay the development of kidney disease.method:Select 110 patients diagnosed as "diabetic nephropathy" in the Department of Nephrology,Affiliated Hospital of Tianjin Academy of Traditional Chinese Medicine,of which 64 patients have diabetic nephropathy with hyperuricemia.Collect and record the basic conditions of patients,clinical indicators,TCM syndromes,and use Excel software to make statistics on TCM symptoms,TCM syndrome types and clinical indicators of patients with diabetic nephropathy,and establish a database.Analyze data through SPSS software,and use pearson chi-square test to analyze the distribution of TCM syndrome types in 64 patients with diabetic nephropathy and hyperuricemia in different clinical stages;use multiple linear regression to analyze metabolic indicators such as blood glucose levels,blood uric acid levels,and blood lipids And the correlation between blood pressure and renal function indexes;the analysis of variance method was used to study the difference between the renal function indexes of different TCM syndrome types.P<0.05 was considered statistically significant.result:1.Distribution of TCM syndrome types: Statistics of TCM syndrome types of 64 diabetic nephropathy patients with hyperuricemia.The main deficiency syndrome is mainly due to deficiency of both qi and yin,and the main evidence is mainly blood stasis syndrome and blood stasis syndrome.The main types are damp turbidity and blood stasis syndrome and damp-heat and blood stasis syndrome.2.In 110 cases of diabetic nephropathy,glycosylated hemoglobin was correlated with 24-hour urine protein quantification and glomerular filtration rate,which was statistically significant(P<0.05).3.64 cases of diabetic nephropathy patients with hyperuricemia had a correlation between serum uric acid level,24-hour urine protein quantification and glomerular filtration rate(P<0.05).And as the blood uric acid level gradually increased,the 24-hour urine protein quantification also increased,and the glomerular filtration rate decreased,which was statistically significant(P<0.05).4.In 64 patients with diabetic nephropathy and hyperuricemia,the correlations between triglycerides and 24-hour urine protein quantification and glomerular filtration rate were not statistically significant(P>0.05);cholesterol and 24 hours The quantitative correlation of urine protein was not statistically significant(P>0.05),and the correlation between cholesterol and glomerular filtration rate was statistically significant(P<0.05).5.In terms of blood pressure,the correlation between blood pressure and 24-hour urine protein quantification and glomerular filtration rate is not statistically significant(P>0.05).6.The 24-hour urine protein quantification of 64 patients with diabetic nephropathy and hyperuricemia was not statistically significant in the various syndrome types of the deficiency syndrome(P>0.05);the glomerular filtration rate was from high to low The corresponding TCM syndrome types in turn are liver and kidney yin deficiency syndrome-lung and kidney qi deficiency syndrome-qi and yin deficiency syndrome-spleen and kidney qi deficiency syndrome-yin and yang deficiency syndrome,with statistical significance(P<0.05).Conclusions.1.The main deficiency syndrome of the patient is Qi and Yin deficiency,and the main evidence is blood stasis syndrome.The main blood stasis syndrome is damp turbidity and blood stasis syndrome and damp-heat and blood stasis syndrome.2.Blood glucose levels and blood uric acid levels are correlated with 24-hour urine protein quantification and glomerular filtration rate in patients with diabetic nephropathy and hyperuricemia;cholesterol is correlated with glomerular filtration rate.3.During the progression of nephropathy in patients with diabetic nephropathy and hyperuricemia,the main deficiency of TCM syndromes is based on liver and kidney yin deficiency syndrome-lung and kidney qi deficiency syndrome-qi and yin deficiency syndrome-spleen and kidney qi deficiency syndrome-yin and yang deficiency syndrome For the development trend.Clinically,the development of diabetic nephropathy combined with hyperuricemia can be delayed by controlling blood sugar,blood uric acid,and blood lipid levels.The treatment of replenishing qi and nourishing yin,tonifying the kidney and promoting blood circulation may be an effective method for the treatment of diabetic nephropathy with hyperuricemia.
