| Objective:In this study,the efficacy of awn needle in the treatment of dyspepsia with mixed cold and heat after stroke was studied,to verify the efficacy of acupuncture treatment,To investigate the efficacy and safety of awn needle in the treatment of dyspepsia with mixed cold and heat after apoplexy.At the same time,to explore the differences in clinical efficacy of awn needle and filiform needle acupuncture in the treatment of this sickness,for providing a new treatment idea and method for this disease.Methods:Sixty patients who met the stangard were divided into two groups,with30 patients in awn needle group and 30 patients in filiform group.Before treatment,all patients were given health education,and basic medical treatment for stroke and various basic diseases,such as lowering blood lipid,blood glucose and blood pressure.At the same time,they were also given Xingnao Kaiqiao acupuncture treatment.On that basis,we regare Xin Kai Ku Jiang,and He Wei Kai Pi as the treatment principle to treat the dyspepsia with cold and heat after stroke.Acupoint selection of the two groups was consistent,there were the main acupoints:Zhongwan,Liangmen(double),Tianshu(double)and the deputy acupoints,which including Zusanli(double),Gongsun(double)and Neiting(double).In the awn needle group,the patients in main acupoints were treated with awn needle,and the deputy acupoints of patients were treated with filiform needle.In the meantime,the above acupoints were treated with filiform needle in the filiform needle group..Acupuncture was kept for30 min each time,once a day,6 times a week,for 2 weeks.After the treatment,the self-designed TCM symptom scale of mixed cold and heat,quality of life index(QOL)scale,Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)were used for scoring.Observing the score change of TCM symptom,single symptom and QOL before treatment,1 week and 2 week.And observing the score change of HAMA and HAMD before treatment and after treatment.After the end,data were collected and a database was established.All the test data were statistically analyzed by IBM-SPSS26.0.Results:1.There was a difference in therapeutic grade between those two groups(P < 0.05).The total effective rate of the awn needle group(96.7%)was evidently higher than the control group(80.0%),(P < 0.05).2.After 1 week and 2 week treatment,the TCM symptom scores have a evidently decline,comparing with treatment before(P < 0.01).The scores in the awn needle group were significantly lower than the filliform needle group at the same time point(P < 0.01).3.1 week of treatment compared with before treatment,flatulence and stomachache、anorexia 、 dry mouth mouth bitter 、 noisy 、 nausea in the awn needle group was significantly improved(P < 0.01).The symptoms of vomiting and bowel in the awn needle group were improved compared with those before treatment(P < 0.05).There was no statistical significance in the score of loose stool symptom in those two groups(P>0.05).In the filiform needle group,the symptoms of flatulence and stomachache、anorexia and nausea were evidently relieved(P < 0.01),the symptoms of dry mouth bitter mouth and noisy were improved(P < 0.05),the symptoms of vomiting,bowel and loose stools have no improvement(P > 0.05),comparing with those before treatment.After treatment,the symptom scores of flatulence and stomachache,dry mouth bitter mouth and noisy in the awn needle group were significantly lower than filliform needle group(P < 0.01).There was no evident difference in the scores of anorexia,nausea,vomiting,bowel sounds and loose stools between the two groups(P > 0.05).4.Compared with before treatment,all single symptoms of TCM in the awn needle group were significantly improved after 2 weeks of treatment(P < 0.01).Compared with before treatment,the symptoms of flatulence and stomachache,anorexia,dry mouth bitter mouth,noise,nausea,vomiting and bowel noise in the filliform needle group were significantly improved(P < 0.01),and the symptoms of loose stool were improved(P < 0.05).After 2 week treatment,the scores of flatulence and stomachache and dry mouth bitter mouth in the awn needle group were significantly lower than filliform needle group(P < 0.01),the scores of nausea,noise,bowel sounds and loose stools were lower than filliform needle group(P < 0.05),the scores of nausea and vomiting were no difference with filliform needle group(P > 0.05).5.Quality of life score:QOL scores in both groups were significantly declined after 1week and 2 weeks of treatment,comparing with those before treatment(P <0.01).After 1 week of treatment,QOL score of awn needle group was higher than filliform needle group(P < 0.05),it was lower than that group(P < 0.01)after 2 week.6.Mental status score:After treatment,HAMA and HADA scores in both groups were evidently lower than those before treatment(P < 0.01),HAMA and HAMD scores in awn needle group were significantly lower than filliform needle group(P <0.01).7.Safety evaluation: There were 2 cases of adverse reactions in the awn needle group.1 case is a slight subcutaneous hemorrhage,1 case is a syncope.There were 2 cases in the filliform needle group,including 2 syncope.For subcutaneous hemorrhage,help patient to slight pressure for a while after bleeding stopped,subcutaneous small bruise appeared,symptoms subsided after 2-3 days.For syncope,ask the patient to lie down and take warm boiled water orally.After resting for a while,the symptoms subsided.The incidence of adverse reactions in both groups was 6.67%,it makes no influence about test progress and test results.Conclusion:1.Both awn needle acupuncture and filiform needle acupuncture can markedly improve the clinical symptoms of patients,but the comprehensive effect of awn needle acupuncture is dramatically better than filiform needle acupuncture.2.The curative effect of awn needle acupuncture on the improvement of single symptoms of traditional Chinese medicine such as flatulence and stomachache,dry mouth bitter mouth and noise is obviously better than that of filiform needle acupuncture,and the effect is quick.3.Both awn needle acupuncture and filiform needle acupuncture can improve the quality of life of patients.The improvement of awn needle acupuncture is better than that of filiform needle acupuncture,and it shows its advantage after 2 weeks of treatment.4.Both awn needle acupuncture and filiform needle acupuncture can improve the symptoms of anxiety and depression in patients.The effect of awn needle acupuncture is better than that of filiform needle acupuncture,and it can adjust the mentality of patients and strengthen the confidence of patients to cure.5.The curative effect of awn acupuncture in the treatment of dyspepsia with mixed cold and heat after apoplexy is definite,with few adverse reactions,no cases of falling off,safe and reliable,simple acupoint selection,and no toxic and side effects of drugs,which is worthy of clinical promotion. |
