Objective: To explore the effect of Qingshen granule on the levels of serum adhesion molecules and chemokines IL-8,and to investigate the changes of MCP-1 in patients with chronic renal failure of damp heat syndrome.Methods: 68 cases were 34 in the control group and 34 in the study group.The experimental group was treated with western medicine basic treatment + Qingshen granule + traditional Chinese cleaning medicine enema,and western medicine was used in the control group basic treatment + traditional Chinese medicine enema.The total time is three months.The clinical result of the treatment group and the control group on patients with chronic renal failure,and the changes of plasma P-selectin,E-selectin,VCAM-1,ICAM-1,serum IL-8 and MCP-1 levels before and after treatment were observed.Results: 1.The difference of clinical results: the dominant rate of the drug group was82.35%,and that of the blank group was 55.88%.(P<0.05)2.The difference of TCM syndrome results: the dominant rate was 82.53% in the medication group and 53.94% in the blank group,the difference was significant(P <0.05).3.Renal function(Scr,e GFR): before treatment,there was no important otherness in renal function between the two groups(P > 0.05).All the time for 3 months,Scr in the treatment group decreased significantly(P < 0.01),e GFR level increased significantly(P < 0.01),a comparison between the two,Scr level declined,e GFR level raised,the otherness was meaningful(P < 0.01);there was no meaningful otherness in renal function in the control group before and after treatment(P > 0.05).4.Comparison of HGB and Alb levels: before treatment,there was no significant difference in HGB and Alb levels between the two groups(P > 0.05).After three weeks of intervention,the levels of HGB and Alb in the two groups were significantly increased(P < 0.01);after intervention,the levels of HGB and Alb in the treatment group were significantly higher than those in the other group(P < 0.01).5.Comparison of the levels of adhesion molecules and chemokines:before intervention,there was nomeaningful otherness in the levels of P-selection,E-selection,VCAM-1,ICAM-1,IL-8 and MCP-1 between the two groups(P > 0.05);after treatment,the levels of P-selection,E-selection,VCAM-1,ICAM-1,IL-8 and MCP-1 in the intervention group were obviously less than those in the other group(P < 0.01).After intervention,the levels of P-selection,E-selection,VCAM-1,ICAM-1,IL-8 and MCP-1in the intervention group were obviously less than those in the other group(P < 0.01).6.Safety instructions: after 3 months of regular dose treatment,the conventional tests of blood,urine,feces,and related biochemical indexes(alt,AST,etc.),12 lead electrocardiogram and other examinations in the treatment group had no significant changes compared with those before treatment.Conclusion: 1.Qingshen granule can effectively alleviate the clinical manifestations of CRF patients with damp heat syndrome,renal function recovered,and effectively improve anemia and nutritional status.2.The levels of P-selection,E-selection,VCAM-1,ICAM-1,IL-8 and MCP-1 were increased in CRF patients with damp heat syndrome.Qingshen granule could down regulate the increased levels of P-selection,E-selection,VCAM-1,ICAM-1,IL-8 and MCP-1.3.During the treatment,no adverse drug events occurred.After the intervention of Qingshen granule,the safety index is basically consistent with that before treatment,which proves that the safety of Qingshen granule is reliable.Objective: by observing the effect of serum containing Qingshen Granule on lipopolysaccharide(LPS)to induce Human umbilical vein endothelial cell adhesion molecule(P-selectin、E-selectin、VCAM-1、ICAM-1),Chemokines((IL-8、MCP-1)and markers of endothelial dysfunction.It is also aim to investigate the intervention effect of Qingshen Granule on endothelial cell injury.Methods: Randomly assign subculture HUVEC into 5 different groups: Normal control group,LPS group,Qingshen granule low dose group,Qingshen granule medium dose group and Qingshen granule high dose group.Cell adhesion test: HUVEC and its VCAM-1、ICAM-1 level.Western blot method to test the level of protein content on E-selectin、VCAM-1、ICAM-1、IL-8、MCP-1、eNOS、ET-1.RT-PCR method to test the expression level of HUVEC on: P-selectin、E-selectin、VCAM-1、ICAM-1、eNOS、ET-1 mRNA.Immunofluorescence assay to test HUVEC: IL-8,MCP-1,eNOS and ET-1protein expression.It also aim to study the effect of Qingshen Granule on the expression of cytokines.Results:(1)cell adhesion rate comparison: compared with the normal group,the cell adhesion rates of VCAM-1 and ICAM-1 induced by LPS were significantly increased(P< 0.05);compared with the LPS group,the cell adhesion rates of VCAM-1 and ICAM-1in the drug intervention groups were significantly decreased(P < 0.05);compared with the different dose groups of Qingshen granule,the cell adhesion rates of VCAM-1 and ICAM-1 in the high dose group of Qingshen granule were significantly decreased(P <0.05)The adhesion rate of am-1 and ICAM-1 was the lowest(P < 0.05).(2)Comparison of the expression of adhesion molecules,chemokines and endothelial cell injury markers(Western Blot method): compared with the normal group,the expression of E-selectin,VCAM-1,ICAM-1,IL-8,MCP-1,ET-1 and eNOS expression were significantly increased(P < 0.05).Compared with LPS group,the expression of E-selectin,VCAM-1,ICAM-1,IL-8,MCP-1,ET-1 in the drug intervention group decreased significantly,and eNOS expression increased significantly(P The results showed that the improvement of Qingshen granule was the most obvious in the high dose group,and the difference was statistically significant(P < 0.05).(3)Comparison of mRNA levels(RT-PCR): compared with the normal group,the expression levels of P-selectin,E-selectin,VCAM-1,ICAM-1,ET-1 mRNA in human umbilical vein endothelial cells induced by LPS were significantly increased,while the expression levels of eNOS mRNA were significantly decreased(P < 0.05);compared with the LPS group,the expression levels of P-selectin,E-selectin,VCAM-1,ICAM-1,ET-1 mRNA in human umbilical vein endothelial cells induced by LPS were significantly increased The expression of eNOS mRNA was significantly increased(P <0.05);the improvement of high-dose group was the most significant(P < 0.05).Conclusion:(1)LPS can increase the expression of P-selectin,E-selectin,VCAM-1,ICAM-1,IL-8 and MCP-1 in HUVEC,decrease the release of eNOS and increase the secretion of ET-1.(2)Qingshen granule can inhibit the expression levels of P-selectin,E-selectin,VCAM-1,ICAM-1 and chemokines IL-8,MCP-1 in HUVEC induced by LPS,increase the release of eNOS,reduce the secretion of ET-1,reduce the adhesion and chemotaxis between cells,and protect vascular endothelial cells in a dose-dependent manner.
