Objective: To study the risk factors and clinical features of lymph-vascular space invasion(LVSI)in cervical cancer,and to provide guidance for clinical comprehensive diagnosis and treatment of early cervical cancer and postoperative complementary therapy.Methods: A retrospective analysis of 2018.10-2020.10 visits from lanzhou university first hospital,120 cases of early cervical cancer(FIGO2018 IA1 ~ Ⅱ A2)in patients with clinical data,it can be divided into LVSI(+)group and LVSI(-)group,compared two groups of patients with preoperative general situation,laboratory examination indexes and postoperative disease inspection results.Results: Univariate analysis showed that SCC-Ag,ratio of fibrinogen to albumin(FAR),FIGO stage,tumor size,degree of differentiation,depth of tumor interstitial invasion,perinerve invasion,lymph node metastasis and positive surgical margin were closely correlated with positive cervical cancer LVSI(P < 0.05).The multivariate analysis showed that SCC-Ag(OR=1.034,95%CI 1.006-1.064,P=0.017),peri-nerve infiltration(OR=31.243,95%CI 6.070~160.811,P= 0.000)and lymph node metastasis(OR=0.149,95%CI 0.030~0.751,P=0.021)were independent risk factors for LVSI in cervical cancer.Receiver operating characteristic(ROC)curve analysis showed that the area under FAR curve was 0.605.When the best diagnostic threshold was 6.8073,the sensitivity and specificity of predicting cervical cancer LVSI were 81%and 38.7%.When the area under the SCC curve was 0.669 and the best diagnostic threshold was 20.2,the sensitivity and specificity of predicting LVSI of cervical cancer were 70.7% and 62.9%.Conclusion: For cervical cancer patients with high SCC-Ag and FAR values,postoperative perinerve infiltration and lymph node metastasis,the incidence of LVSI is high,so clinical attention should be paid to strengthen the comprehensive evaluation of patients,so as to develop appropriate treatment plan and carry out more rigorous follow-up after the operation.
