| Objective: The purpose of this study was to analyze the level of blood lipid in elderly patients with coronary heart disease,and to study the correlation between non-high-density lipoprotein cholesterol and coronary artery disease in elderly patients with coronary heart disease.Methods: In this study,370 elderly patients with suspected coronary heart disease with clinical symptoms of chest distress and chest pain admitted to the Department of Cardiology,Lanzhou University First Hospital from February 2018 to February 2020 were retrospectively enrolled and underwent coronary angiography.General data,serological indexes,cardiac ultrasound and coronary angiography were collected,and non-high-density lipoprotein cholesterol(non-HDL-C)and Gensini score were calculated.Among them,229 cases were diagnosed as elderly patients with coronary heart disease(age ≥60 years),and the remaining 141 cases were used as control group.The differences of basic data between the two groups were compared to study the influencing factors of coronary heart disease in the elderly.According to the three digits spacing of Gensini score,the elderly CHD group was divided into three groups: mild,moderate and severe.The differences in basic data among the three groups were compared,to study the risk factors of the severity of artery disease.Results: 1.A total of 370 elderly patients were included in this study,of which 229 were diagnosed with CHD and 141 were the control group.The results showed that gender,smoking history and drinking history were different between the senile CHD group and the control group(P <0.05).The levels of TC,non-HDL-C,TG,LDL-C,CREA,Hcy and Neut in senile CHD group were higher than those in control group(P <0.05).The risk factors of coronary heart disease in the elderly were gender,non-HDL-C,TG,Hcy and diabetes mellitus.The risk of coronary heart disease in males was 0.268 times that in females.2.According to Gensini score,the elderly coronary heart disease group was divided into three groups: mild(< 22 points),moderate(22-44points)and severe(> 44points),and the results showed that there were differences in TC,non-HDL-C,TG,LDL-C and LVEF(P < 0.05).The differences between the two groups were further analyzed.TC and non-HDL-C were different in the mild and moderate groups,the mild and severe groups,and the moderate and severe groups(P < 0.05).LDL-C was significantly different between mild and moderate groups,and between mild and severe groups(P < 0.05).TG was only different between mild and severe(P < 0.05).There were differences in LVEF between mild and severe groups,moderate and severe groups(P < 0.05).3.By analyzing the correlation between the different indexes in the three subgroups and the Pearson rating of Gensini branch,TC,TG and Gensini score were positively correlated(r=0.390,r=0.212).Non-HDL-C and LDL-C were also positively correlated with Ginsini score,and the correlation coefficient of non-HDL-C was higher(r=0.457,r=0.268).LVEF was negatively correlated with Gensini score,and the correlation coefficient r was-0.200.4.According to clinical experience and general data results,TC,non-HDL-C,TG,LDL-C and LVEF were included in ordered Logistic regression analysis.The results showed that the risk factors of coronary artery disease in elderly patients with coronary heart disease were non-HDL-C and TG,and the protective factor was LVEF.Conclusion:Non-HDL-C is not only a risk factor for coronary heart disease in the elderly,but also a risk factor for the severity of coronary artery disease. |
