| OBJECTIVEWe analyzed the data on inpatients with reflux esophagitis(RE)diagnosed by endoscopy in the First Hospital of Lanzhou University in the past 2.5 years,hoping to find the incidence characteristics of the RE population,and explore the influencing factors related to the severity of RE,as well as the incidence and risk factors of metabolic syndrome(MetS)in the RE population,so as to provide comprehensive basis and ideas for the prevention and treatment of RE.METHODS1.Collected the data of 634 patients diagnosed with RE by gastroscopy in the First Hospital of Lanzhou University from January 1,2018 to June 30,2020,which included general information,imaging examination,clinical symptoms,endoscopic classif-ication and combined diseases,helicobacter pylori(Hp)infection,laboratory indicators and other items.Then the data was statistically described,and the characteristics of the RE population were discussed.2.Based on the Los Angeles(LA)grading method,RE patients were divided into three groups,LA-A,LA-B,and LA-CD.The demographic information,imaging examinations,endoscopic data and laboratory results of the three groups of RE patients were compared.Single factor analysis and ordered Logistic regression analysis were carried out to screen the factors that affected the severity of RE.3.Dividing these patients into two group based on whether combined with MetS to explore the incidence rate and risk factors of MetS in the RE population.RESULTS1.Among the 634 RE patients,467 cases(73.66%)were males and 167 cases(26.34%)were females,and the ratio of male to female was 2.8:1.The average age was58.47±12.36 years old,and the peak incidence age was 50-59 years old.According to BMI,35 cases(5.52%)of the patients were thin,265 cases(41.80%)were normal,287cases(45.27%)were overweight,and 47 cases(7.41%)were obese.Analysis of the personal history suggested that there were 164(25.87%)smokers and 153(24.13%)drinkers.The top four combined underlying diseases were hypertension,gallbladder disease,fatty liver,diabetes,with 230 cases(36.28%),164 cases(25.87%),140 cases(22.08%)and 125 cases(19.72%)respectively.The common symptoms related to RE were abdominal distension(42.08%),reflux(41.08%),abdominal pain(37.27%),and heartburn(26.05%).2.The 634 cases of RE were classified into four group according to LA classification,which included 339 cases(53.47%)of grade A,234 cases(36.91%)B,42 cases(6.62%)C and 19 cases(3.00%)D.The peak incidence age of grade A and grade B both were 50-59 years old,115 cases and 71 cases respectively.The peak incidence age of grade C and grade D both were over 60 years old,26 cases and 12cases respectively.The main combined diseases of RE patients under gastroscope were chronic atrophic gastritis(243,38.33%),chronic non-atrophic gastritis(119,18.77%),and erosive gastritis(161,25.39%).A total of 218 patients underwent 14C breath test,and 56 cases(25.69%)were Hp(+).3.Among the 634 patients with RE,633 cases have undergone routine blood testing,77 cases have undergone pepsinogen and gastrin testing,the number of patients who have undergone blood biochemical testing and blood tumor markers testing were between 607-634 and 247-485 respectively.4.Through the comparison between the three groups of LA-A,LA-B,and LA-CD,the single factor analysis suggested that gender,age,hiatal hernia,esophageal and duodenal ulcers,chronic non-atrophic and erosive gastritis,RBC,SCC,AFP may be related to the severity of RE.5.The orderly Logistic regression analysis of the factors showed that esophageal hiatal hernia was positively correlated with the severity of RE,while erosive gastritis and chronic non-atrophic gastritis were negatively correlated with the severity of RE.6.Among the RE population,142 patients also exist MetS,and the incidence rate was 22.40%.Hypertension and the decrease of HDL-C were risk factors for MetS in RE population.CONCLUSION1.RE is more common in men,mainly in middle-aged and elderly,the peak incidence age of male is earlier than that of women,and the common basic diseases of patients with RE are hypertension,gallbladder disease,fatty liver and diabetes.2.The classification of RE under endoscopy is more common in grades A and B,and less in grades C and D.The main combined diseases of RE patients under gastroscope are chronic atrophic gastritis,chronic non-atrophic gastritis and erosion gastritis.3.Combined with esophageal hiatal hernia,erosive gastritis and chronic non-atrophic gastritis are the influencing factors for the severity of RE.4.In the RE population,the incidence of MetS is 22.40%.Hypertension and the decrease of HDL-C are risk factors for the incidence of MetS in the RE population. |
