Objective:Discuss intelligent electronic spectroscopy technology(Flexible spectral imaging color enhancement,FICE),magnifying endoscopy(Magnifying endoscopy,ME)combined with FICE(FICE-ME)and blue laser imaging(Blue LASER Imaging,BLI)combined with ME(BLI-ME),The clinical diagnostic value of early gastric cancer(EGC).Methods:The clinical and pathological data of gastric mucosal erosion and roughness suspected to be EGC and Precancerous lesions(PLGC)from January 2017 to December 2020were collected,624 patients were divided into control group(304 cases)and Observation Group(320 cases)by White light endoscopy(WLE)and FICE,the results of endoscopic diagnosis and pathological diagnosis in the two groups were analyzed retrospectively.Selected 158 patients with suspected EGC admitted to this hospital from January 2017 to December 2020 as the research objects.All patients in the group underwent WLE examination as suspected EGC and routine biopsy.After that,FICE-ME was used to treat the microvessels on the mucosal surface of the lesion(Microvascular pattern(MV)and surface microstructure(Microsurfare pattern,MS)were observed twice.Those who were judged to be EGC received endoscopic resection or surgical resection,and some of the removed lesions were sent to pathological biopsy.Retrospective analysis and comparison of the two Endoscope diagnosis results and pathological examination results.A total of 108 suspected EGC patients who underwent WLE examinations and routine biopsy in our hospital from January2019 to December 2020 were selected as the research subjects.Among them,52cases underwent FICE-ME examination,56 cases underwent BLI-ME examination,and 2 types were analyzed.Endoscope diagnosis and pathological examination results.Take pathological diagnosis as the gold standard to evaluate the sensitivity,accuracy and specificity of WLE and FICE for EGC and PLGC;evaluate the sensitivity,accuracy and specificity of WLE and FICE-ME for EGC;compare the sensitivity and specificity of BLI-ME and FICE-ME in the diagnosis of EGC,use Kappa value to test the consistency of the diagnosis results of the two diagnostic methods with the pathological diagnosis results,and use receiver operator characteristic curve(ROC)Analyze the diagnostic efficiency of the two diagnostic methods.Results:Among 624 patients,the diagnostic sensitivity and accuracy of the observation group(87.90%,84.06%)were better than those of the control group(44.32%70.39%,P<0.05),and there was no significant difference in specificity(χ~2=0.03,P>0.05).Among the 158 patients with suspected EGC,the diagnostic sensitivity,specificity and accuracy of WLE and FICE-ME were 51.06%,82.35%,54.43%and 91.48%,76.47%,89.87%,respectively.FICE-ME had high diagnostic sensitivity and accuracy.In WLE(χ~2=56.28,49.39,P<0.05),there is no significant difference in specificity(χ~2=0.18,P>0.05).The sensitivity,specificity and accuracy of BLI-ME and FICE-ME in diagnosing EGC were 95.45%,91.18%,92.86%and 86.36%,90.00%,88.46%,respectively.There was no significant difference in sensitivity,specificity and accuracy(P>0.05),the Kappa values were 0.853 and 0.764,respectively,and the AUC of the two diagnostic methods to diagnose EGC were 0.933 and 0.882(P<0.05).Conclusion:Compared with WLE,FICE has a better diagnostic effect on EGC and PLGC;FICE-ME has higher sensitivity and accuracy in EGC diagnosis than WLE,and has a higher diagnostic value.Both BLI-ME and FICE-ME have high diagnostic accuracy rates for EGC.Both BLI-ME and FICE-ME are suitable for the diagnosis of EGC.The results of the two diagnostic methods are in good agreement with the results of pathological diagnosis.BLI-The diagnostic efficiency of ME is higher,and it is more suitable for precise examination of EGC.
