BackgroundEsophageal cancer is a common malignant tumor.Esophageal squamous cell carcinoma(ESCC)is the main type of esophageal cancer in China,accounting for about90% of all cases.Chemotherapy is one of the most important treatments for advanced esophageal squamous cell carcinoma.Studies have shown that almost all malignant tumors express VEGF extensively,and the up-regulated VEGF binding to its receptor promotes tumor progression by activating related pathways.Studies have shown that paclitaxel combined with cisplatin(TP)regimen can reduce the level of serum VEGF in patients with esophageal squamous cell carcinoma,but the relationship between the level and the short-term efficacy of chemotherapy has not been fully confirmed.ObjectiveTo investigate the relationship between peripheral serum VEGF level and clinicopathological features in patients with esophageal squamous cell carcinoma.To observe the changes of peripheral serum VEGF levels in patients with esophageal squamous cell carcinoma during TP regimen chemotherapy,and to analyze the correlation between VEGF levels and short-term efficacy,in order to provide testable indicators for dynamic evaluation of short-term efficacy of TP regimen.MethodsA selection of 87 newly-treated patients with esophageal squamous cell carcinoma diagnosed by pathological examinations of the First Affiliated Hospital of Xinxiang Medical College from December 2018 to June 2020 as the test group,and 40 healthy persons who had physical examinations during the same period were selected as the control group,and the peripherals of the two groups were compared Serum VEGF level,analyze the relationship between the peripheral serum VEGF expression level and clinicopathological parameters of patients with esophageal squamous cell carcinoma.The receiver operating characteristic curve(ROC)was used to evaluate the diagnostic efficacy of serum VEGF for esophageal squamous cell carcinoma.62 patients in the test group received paclitaxel combined with cisplatin chemotherapy.The peripheral serum VEGF levels of patients with esophageal squamous cell carcinoma were compared before and after 2 cycles of chemotherapy,and the relationship between it and the clinical efficacy was analyzed.Results1.The peripheral serum VEGF level of 260.23(160.68-422.49)pg/m L in the test group was significantly higher than that in the healthy control group(96.49(73.90-151.21)pg/m L)(P(27)0.05).According to the median VEGF level of esophageal squamous cell carcinoma in the experimental group,the patients were divided into two groups: high expression of VEGF(≥260.23 pg/m L)and low expression of VEGF((27)260.23 pg/m L).There was no significant difference between the expression of VEGF in peripheral serum and sex,age,location of lesion,tumor size and T stage in patients with esophageal squamous cell carcinoma(P(29)0.05).The difference was statistically significant with lymph node metastasis,distant metastasis and clinical stage(P(27)0.05).Abnormally high expression of VEGF in peripheral serum of patients with esophageal squamous cell carcinoma is significantly correlated with disease progression2.ROC was used to evaluate the diagnostic efficacy of serum VEGF in esophageal squamous cell carcinoma.The AUC of serum VEGF level in the diagnosis of esophageal squamous cell carcinoma was 0.863,and the best diagnostic cutoff value was 229.74 pg/m L.3.Among the 87 patients with esophageal squamous cell carcinoma in the experimental group,62 patients received systemic chemotherapy with paclitaxel and cisplatin.The expression level of VEGF in peripheral serum of patients with esophageal squamous cell carcinoma after 2-week chemotherapy was 217.38(127.16-354.26)pg/m L significantly lower than that before chemotherapy 259.84(157.43-433.17)pg/m L,The difference was statistically significant(P(27)0.05).4.PD group was defined as ineffective group,CR group SD group and PR group as effective group.There was no significant correlation between serum VEGF level before chemotherapy and short-term chemotherapy efficacy(P(29)0.05).5.After 2 cycles of chemotherapy,the level of serum VEGF in patients with effective chemotherapy was significantly lower than that before chemotherapy(P(27)0.05);After 2 cycles of chemotherapy,the level of serum VEGF in patients with ineffective chemotherapy was significantly higher than that before chemotherapy(P(27)0.05).Conclusion1.The level of peripheral serum VEGF in patients with esophageal squamous cell carcinoma was higher than that in healthy controls,and the level of peripheral serum VEGF in patients with esophageal squamous cell carcinoma was positively correlated with lymph node metastasis,distant metastasis and clinical stage.2.During the chemotherapy of TP regimen in patients with esophageal squamous cell carcinoma,the change level of serum VEGF was correlated with the short-term efficacy of chemotherapy,and the level of VEGF may be an index to dynamically evaluate the short-term efficacy of TP regimen.
