| Objective:to investigate the diagnostic value of serum tumor markers CEA,SCCA,CYFRA21-1 and NSE in postoperative recurrence and metastasis of lung cancer.Methods:We retrospectively analyzed 250 patients with lung cancer who were admitted to the Department of Thoracic Surgery,Affiliated Hospital of Qinghai University from January 2014 to January 2018.Among them,170 patients underwent radical surgery,and 80 patients had recurrence within 3 years after surgery.Metastasis patients,90 patients who did not relapse within 3 years after surgery;compare the serum CEA,SCCA,CYFRA21-1,and NSE levels of the two groups respectively,and analyze the sensitivity and specificity of four tumor markers in the diagnosis of lung cancer.The contrast is different Serum CEA,SCCA,CYFRA21-1 and NSE levels in lung cancer patients with pathological type and clinical stage.At the same time,we collected 30 middle-advanced lung cancer patients undergoing(Pemetrexed +platinum drugs)chemotherapy,30 targeted therapy patients,and 18 patients undergoing(Pemetrexed + platinum drugs)chemotherapy + PD-1/PD-L1 immune checkpoint inhibitor therapy.Compare the serum CEA,SCCA,CYFRA21-1,NSE of the chemotherapy group and the chemotherapy + PD-1/PD-L1 immune checkpoint inhibitor treatment group,the chemotherapy group and the targeted therapy group(12weeks)after 4 chemotherapy cycles.Level.Then analyze the relationship between serum tumor marker levels and postoperative recurrence and metastasis of lung cancer.Results: among the four tumor markers,CEA,SCCA and CYFRA21-1 had high sensitivity and specificity for postoperative recurrence and metastasis of lung cancer.The sensitivity and specificity of CEA for predicting tumor recurrence were 75.0%and 93.3%,respectively.The sensitivity and specificity of SCCA for predicting tumor recurrence and metastasis were 61.3% and 96.7%,respectively.The sensitivity and specificity of Cyfra21-1 for predicting tumor recurrence and metastasis were 73.8%and 97.8%,respectively.By adjusting the critical values of these three tumor markers,the sensitivity of tumor markers to the prediction of postoperative tumor recurrence can be improved.There were significant differences in the effects of postoperative chemotherapy and chemotherapy combined with PD-1/PD-L1 immunocheckpoint inhibitor therapy on the levels of CEA,SCCA and CYFRA21-1 in middle and advanced non-small cell lung cancer.Conclusion: among the four tumor markers,CEA,SCCA and CYFRA21-1 have high specificity and sensitivity in predicting postoperative recurrence and metastasis of lung cancer.Combined detection of CEA,SCCA and CYFRA21-1 can significantly improve the sensitivity of tumor markers in predicting recurrence of lung cancer.Different postoperative treatments for lung cancer will affect the levels of CEA,SCCA and CYFRA21-1. |
