| Objective:To describe the pathological mechanism and MRI manifestation of liver perivascular lucency(PVL),to explore the correlation with liver diseases,and to evaluate the clinical significance of PVL.Methods:The MRI findings of PVL in 197 patients with liver-related diseases(mainly chronic hepatitis B,cirrhosis,and liver tumors)in our hospital from January 2019 to January 2020 were retrospectively analyzed.To investigate the correlation between PVL and liver diseases,the MRI data of the patients with plain and enhanced upper abdomen were studied.Results:A total of 197 cases were included in this study,including 111 in chronic hepatitis B group,57 in cirrhosis group and 29 in liver tumor group.Results :(1)The PVL detection rate was 32.4% in patients with chronic hepatitis B,31.0% in patients with liver tumor,and 40.4% in patients with cirrhosis.(2)The incidence of PVL in patients with chronic hepatitis B increased as the severity of hepatitis increased,showing a positive correlation,and the difference was statistically significant(P=0.006<0.05);as the grade of chronic hepatitis B worsened,the signs of PVL showed The effective rate of treatment decreased gradually,with statistical difference(P=0.006<0.05).(3)The incidence of PVL in patients with cirrhosis did not change with the increase of cirrhosis grading score(P>0.05).There was no statistical difference in PVL effective rate among groups with different cirrhosis scores(P=0.608>0.05).(4)PVL was found in 7 of 9 patients with hepatic tumor obstruction(77.8%),and the incidence of PVL was much higher than that in patients with tumor without portal invasion(10.0%),and the difference was statistically significant(P < 0.001).Conclusion:The MRI distribution and manifestations of PVL have corresponding characteristics in different liver diseases.PVL can indirectly reflect the changes in the function and structure of the liver lobules,and to a certain extent can reflect the severity of chronic hepatitis B and whether the hepatic portal area of patients with liver tumors has been violated.Therefore,it has reference value for the evaluation of the severity and prognosis of chronic hepatitis B disease,and the diagnosis of whether liver tumors invade the hilar area. |
