| Objective:To explore the adjustment of the compression intensity of the radial artery compressor and the best decompression plan for patients after transradial artery PCI based on dynamic blood pressure,so as to reduce the incidence of complications in patients after transradial artery PCI,and improve the quality of medical care and patient comfort.Method:The convenience sampling method was used to select 115 patients who underwent PCI via radial artery in the cardiovascular department of a tertiary A hospital in Nanchang from October 2019 to October 2020 as the research objects.Among them,55 cases were in the test group and 60 cases were in the control group.The plan of the control group adopts the decompression plan made by the department.That is to say,the compression intensity of the radial artery compressor after the radial artery path PCI is used to pressurize and stop bleeding according to experience.After 2 hours,the patient’s radial artery compressor is relaxed one circle(the pressure generated by a spiral compressor is 62 mm Hg,1mm Hg=0.133 k Pa),the patient’s radial artery compressor was relaxed again 4 hours after the operation,the compressor was completely released 6 hours after the operation,and the radial artery compressor was completely removed 24 hours after the operation.Observe whether the patient has complications such as bleeding and swelling at the puncture site,and record the decompression Time,decompression pressure value,complications and their degree.The protocol of the experimental group was based on the dynamic blood pressure to decompress the radial artery.That is,the nurse in the intervention room immediately after the radial artery path PCI measures the patient’s arterial blood pressure.The interventional doctor adjusts the compression intensity of the radial artery compressor to 30 mm Hg higher than the patient’s systolic pressure according to the patient’s systolic pressure value,and records the compression intensity and compression of the radial artery compressor Time and send the patient back to the ward.If the patient has bleeding or swelling of the limb on the surgical side immediately after the operation,it will be treated according to the elimination criteria.Two hours after the operation,members of the research team used the bedside ECG monitor and started to use the patient’s blood pressure until the observation time point,and the average value was taken.At the same time,the blood pressure was measured once 1 min before the radial artery decompression,and the diastolic blood pressure of the two was compared.The high value is used as the compression intensity value;4hours after the operation,members of the research team transferred the blood pressure from the bedside ECG monitor to the observation time point,and calculated the average diastolic blood pressure.At the same time,the blood pressure was measured1 min before the radial artery decompression.,Compare the diastolic pressure of the two,take 1/2 of the high value as the compression intensity value;completely relax the radial artery compressor 6 hours after the operation,and completely release the radial artery compressor 24 hours after the operation.Every time the compression intensity is adjusted,a person at the bedside will take care of him for 10-15 minutes,and observe the bleeding at the puncture site.The compression intensity was adjusted every 2 hours after operation in both groups,and the bleeding,pain,swelling,etc.and their degree were compared between the two groups.Result:1.Adjusting the compression intensity of the radial artery compressor according to the dynamic blood pressure can effectively stop bleeding and reduce the pain of the postoperative limb.2.Adjusting the compression intensity of the radial artery compressor according to the dynamic blood pressure can reduce the incidence of postoperative limb swelling in patients,and effectively reduce the degree of postoperative swelling.Conclusion:It is safer to adjust the compression intensity of the radial artery compressor according to the dynamic blood pressure after PCI through the radial artery path,and the hemostatic effect is more reliable,which can reduce the incidence of pain and swelling complications in patients. |
