Objective:Through the changes of serum concentrations of oxygen free radicals and melatonin in patients with ISSNHL before and after treatment,to study the effect of oxidative stress on the pathogenesis of ISSNHL,and whether melatonin has a protective effect on the hearing of patients with ISSNHL.Methods:In this study,43 patients with ISSNHL in the department of otolaryngology of our hospital from July 2020 to December 2020 were selected as the sudden deafness group,and 15 healthy volunteers with matched age and normal hearing were selected as the control group.The concentrations of serum melatonin,oxygen free radicals and total antioxidant capacity of the two groups were measured,and the concentrations of melatonin and oxygen free radicals in patients with sudden deafness were compared between the sudden deafness group and the control group.the changes of melatonin and oxygen free radicals before and after treatment were compared to analyze whether oxygen free radicals were involved in the pathogenesis of ISSNHL and whether melatonin had a protective effect on the hearing of patients with ISSNHL.To analyze the relationship between oxidative stress and prognosis of patients with ISSNHL.Results:Before treatment,43 patients with ISSNHL and 15 healthy controls had serum melatonin levels(1237.77 ±742.35VS1841.87 ±1336.57),oxygen free radicals(4.42±4.40VS2.30 ±0.59)and total antioxidant capacity(0.67 ±0.08VS0.66 ±0.08).(2)43patients with ISSNHL were divided into effective group and ineffective group.Before treatment,melatonin(1400.83 ±784.89VS1095.97 ±689.08),oxygen free radical(3.05 ±1.81VS5.62 ±5.56)and total antioxidant capacity(0.68 ±0.08VS0.66 ±0.08)were compared.(3)before and after treatment,melatonin(1279.83 ±691.29VS425.24±69.14),oxygen free radicals(4.50 ±5.02VS3.32 ±2.54)and total antioxidant capacity(0.69 ±0.08VS0.83 ±0.09)were compared in 31 patients with ISSNHL.(4)in 31 patients with ISSNHL,melatonin(425.24 ±69.14VS1841.87 ±1336.57),oxygen free radicals(3.06 ±2.18VS2.30 ±0.59)and total antioxidant capacity(0.83±0.09VS0.66 ±0.08)were compared with healthy controls.Conclusion:(1)oxygen free radicals may be involved in the occurrence and development of ISSNHL,and the higher the concentration of oxygen free radicals,the worse the prognosis of patients with ISSNHL.(2)melatonin plays a positive role in improving the hearing of patients with ISSNHL.the prognosis of patients with high concentration of melatonin is good.(3)after systematic treatment,the level of oxygen free radicals decreased to normal and the ability of antioxidation increased,but only some patients’ hearing was improved,suggesting that there are other factors affecting hearing.
