| Objective:Osteocalcin(OC)and alkaline phosphatase(ALP)are markers of bone formation,which can specifically reflect the function of osteoblasts at different mature stages.They are the key indicators of bone metabolism in children’s growth.In this paper,we compared the differences of serum OC and ALP levels in children with growth hormone deficiency(GHD),idiopathic short stature(ISS)and healthy children,understood the level of bone metabolism in children with short stature and the changes after treatment.The correlation between Insulin-like growth factor-1(IGF-1)and height velocity(HV)after Recombinant human growth hormone(rhGH)treatment for 12 months was analyzed,Objective to study whether bone metabolism indexes(OC,ALP)can be used as one of the indexes to determine the efficacy of rhGH.Methods:From August 2018 to March 2021,short stature children diagnosed in Department of Endocrinology,Metabolism and Genetics,Jiangxi Provincial Children’s Hospital were selected as the experimental group.Among them,50 cases of GHD(Group A)and 39 cases of ISS(Group B)were the experimental group.According to whether there is breast development or testicular enlargement(volume>4ml),they were divided into A1 group and B1 group(without breast development or testicular enlargement),A2 group and B2 group(with breast development or testicular enlargement).The bone age,Pituitary magnetic resonance imaging,IGF-1,OC and ALP were measured;The levels of OC,ALP and IGF-1 in the experimental group were measured after rhGH treatment for 3 months,6 months,9 months and 12 months,HV was monitored after treatment.Forty seven healthy children who received physical examination in the Department of child health care in the same period were selected as the control group.According to whether there was breast development or testicular enlargement(volume>4ml),they were divided into C1 group(without breast development or testicular enlargement),C2 group(with breast development or testicular enlargement),OC and ALP were determined.The differences of serum OC and ALP among GHD,ISS and healthy children were compared,The levels of serum OC,ALP and IGF-1 were compared before and after treatment.HV of the experimental group was calculated.The relationship between OC and ALP,IGF-1,and the relationship between OC,ALP and HV were analyzed.The relationship between the changes of serum OC,ALP and HV after 12 months of treatment were analyzed.Results:The results were as follows: 1.The serum OC levels of children with GHD and ISS were lower than those of healthy children(P<0.05);2.The serum OC level of GHD children was lower than that of ISS children(P<0.05);3.The levels of OC,ALP and IGF-1 in serum of children with GHD and ISS after rhGH treatment were higher than those before treatment(P<0.05),and the biggest change was in the three months before treatment;4.The level of OC was positively correlated with ALP and IGF-1 after rhGH treatment;5.There was a positive correlation between the changes of serum HV after 12 months of rhGH treatment and the level of serum OC 3 months after treatment.Inconclusion:The serum levels of OC and ALP in children with short stature were lower than those in healthy children;Rh GH treatment could improve the serum levels of OC and ALP in children with GHD and ISS;The change of serum OC Level 3 months after treatment can be used as an index to evaluate the therapeutic effect of rhGH in children with short stature. |
