Objective Through the collection of preoperative blood routine and postoperative pathological results of patients undergoing hilar cholangiocarcinoma radical resection in our department in the past 10 years.Use statistical methods to study the relationship between preoperative neutrophil to lymphocyte ratio(NLR)and platelet to lymphocyte ratio(PLR),clinicopathological characteristics and T staging of hilar cholangiocarcinoma,and whether preoperative NLR and PLR contribute to T staging has predictive value.Methods The data of 100 patients with hilar cholangiocarcinoma diagnosed pathologically in the Department of hepatobiliary surgery,the First Affiliated Hospital of Bengbu Medical College from January 2010 to December 2019 were retrospectively analyzed.The optimal cut-off value of NLR and PLR was determined by ROC curve.The relationship between preoperative NLR and PLR,pathological characteristics and T stage of hilar cholangiocarcinoma was determined by univariate and multivariate analysis.The area under the ROC curve is used to confirm whether preoperative NLR and PLR have predictive value for T staging.Results According to the ROC curve,the best cut-off values of NLR and PLR are4.12 and 184.56,respectively.Univariate analysis showed that preoperative NLR was correlated with tumor location(χ2=5.622 P=0.018)and T stage(χ2=8.501 P=0.004),and was related to gender,age,degree of differentiation,tumor size,TNM stage and lymph node metastasis no correlation,P>0.05;Preoperative PLR is correlated with T stage(χ2=9.350 P=0.002),tumor size(χ2=4.720 P=0.030)and lymph node metastasis(χ2=6.692 P=0.010),and there was no correlation with gender,age,degree of differentiation,tumor location and TNM stage P > 0.05.Multivariate analysis showed that preoperative NLR was associated with T staging(OR=2.999,P=0,026)and tumor location(OR=0.346,P=0.032);preoperative PLR was correlated with T stage(OR=3.635,P=0.007)and tumor size(OR=1.728,P=0.031),but had no correlation with lymph node metastasis,P>0.05.The area under the ROC curve of NLR and PLR before surgery were 0.811(95% CI 0.727-0.894,P <0.01)and 0.794(95% CI0.708-0.880,P <0.01),indicating that preoperative NLR and PLR have predictive value for different T stages.Conclusion Preoperative NLR and PLR are simple,convenient,and practical detection indicators,which can be used to evaluate the pathological characteristics of hilar cholangiocarcinoma before surgery,and have predictive value for different T stages.
