| Objective:HR-MRI VWI was used to analyze the degree of intracranial large vessel stenosis,plaque location and plaque stability,and to evaluate the role of HR-MRI VWI in the Chinese ischemic stroke subclassification(CISS).So as to provide the basis for the precise treatment of ischemic stroke.Methods:In June 2018-June 2020 in Ningxia hui autonomous region people’s hospital of 522 patients with acute ischemic stroke in neurology,after admission to collect the general information of patients,on the basis of medical history,symptoms,signs,NIHSS score,laboratory examination and conventional imaging examination data preliminary CISS parting line,ruled out clear of 47 patients of cardiac stroke,the other into the group of patients improve HR-MRI VWI(T1WI_VISTA T2WI_VIST T1W_VISTA + C,SWI)check with 154 cases,One case was excluded due to poor image quality,and the results of CISS classification of 153 cases before and after HR-MRI VWI examination were analyzed.Results: A total of 522 patients with acute ischemic stroke who met the eligibility criteria were included in this study.Among them,there were 336 males and 186 females,ranging in age from 25 to 89 years old,with an average of(65.82±12.74)years old.According to the patient’s laboratory tests(blood lipids,blood glucose,homocysteine,uric acid,etc.)and related auxiliary examination(24 hours ambulatory blood pressure,24 hours dynamic electrocardiogram,ultrasonic cardiography,cervical artery ultrasound,TCD and CTA and MRA)as a result,the preliminary CISS parting line: artery atherosclerosis(LAA):in 176 cases(33.72%),cardiac stroke(CS): 47 cases(9.00%),wear the arterial disease(PAD):150 cases(28.74%),other etiological type(OE):8 cases(1.53%),uncertain cause(UE): 141 cases(27.01%).There was no significant difference in age,gender,hypertension,diabetes,previous stroke history,smoking history and alcohol consumption history among the enrolled patients(P>0.05).Among 153 patients,93 cases(60.78%)had consistent classification before and after HR-MRI VWI examination,while 60 cases(39.22%)had inconsistent classification.HRMRI VWI artery atherosclerosis after checking the proportion increased 34.64%,perforators artery lesion type proportion dropped 25.49%,unknown reason proportion was reduced by11.76%;HR-MRI VWI CISS classification rate before and after comparison,?2 = 47.649,P= 0.000 difference was statistically significant.Conclusion:1.After HR-MRI VWI examination,the proportion of patients diagnosed as LAA type and OE type increased,while the proportion of patients diagnosed as PAD type and UE type decreased;2.HR-MRI VWI has a high degree of recognition for intracranial artery lesions;3.HR-MRI VWI is helpful for more accurate CISS classification and provides basis for standardized treatment of ischemic stroke. |
