Research PurposeBased on the theory of circadian rhythm mechanism,Dietary management scheme was established for the enteral nutrition transition period of critically ill patients,To achieve the continuous management of enteral nutrition treatment for critically ill patients,so as to reduce the risk of gastrointestinal events such as abdominal distension and night hunger in the early stage of oral feeding.Research MethodsThis research adopt literature research method and expert meetings, building the enteral nutrition management scheme in transition period for critically ill patients, and extract the city of a tertiary grade A general hospital between December 2019 and December 2020,admitted 66 patients who met inclusion and exclusion criteria of nonrandomized controlled studies during this period, each period of eligible patients using random number table method were randomly divided into control group and observation group.Control group patients in the conventional nursing measures before the end of enteral nutrition therapy, observation group use diet intervention management scheme, compared two groups of patients through the mouth to eat early abdominal distention score, bowel sounds auscultation, promote gastric dynamic drug application, hungry at night, feeding events will not success rate and patients’ satisfaction score if there is a statistically significant.Research ResultsAnalysis of variance of repeated measurements showed that there was statistical significance in the time effect(P < 0.001),indicating that there was statistical difference between the two groups at different measurement time points. There was no statistical significance in the interaction effect(P > 0.05),indicating that the time factor did not affect the change of the research results. There was statistical significance in group effect(P < 0.001),indicating that transitional diet management program can effectively alleviate abdominal distension symptoms in critically ill patients at the early stage of oral feeding. The intestinal sounds assessment results showed that there was statistical significance between the two groups in the early stage of oral feeding(P < 0.05),and the disorder degree in the control group was significantly lower than that in the observation group.Promoting gastric dynamics drug application, hunger event happening at night and feeding conditions to make the 1 day after intervention compared with cumulative frequency differences are statistically significant(P < 0.05),and the number of control group is lower than the observation group, within the 2 days and 3 days after intervention differences comparing with no statistical significance(P > 0.05).After the end of the study, the two groups of patients on the diet management program satisfaction survey found that the two groups of patients in the nursing operation of the difference was statistically significant(P < 0.05),there was no statistical significance in psychological nursing and other aspects of the difference(P > 0.05).Research ConclusionBased on the mechanism of circadian rhythm related theoretical basis, constructed for critically ill patients with enteral nutrition transition dietary management scheme, can effectively reduce the patient’s risk of early gastrointestinal events occurring through the mouth to eat,to optimize the management of enteral nutrition in the scheme, the purpose of enteral nutrition therapy continuity, high management,at the same time, for clinical medical staff medical decision-making to provide a certain reference value.
