| Blackground: With the increasing trend of human aging,stroke has become one of the diseases that seriously harm the health of middle-aged and old people all over the world.In China,stroke incidence rate beneficial to the people’s livelihood and the high morbidity rate,high mortality rate and annual growth cost have caused enormous social and economic burden to China,and become an important public health problem that seriously affects the national economy and people’s livelihood.In order to strengthen the prevention and treatment of regional stroke,China started the construction and evaluation of stroke centers.In the construction of advanced stroke centers in regional hospitals,it is an important goal to improve the construction quality indicators.The comprehensive application of scientific management tools in the construction of advanced stroke center is less,which needs further study.Objective: To investigate the clinical application and effect of management tools in the construction of advanced stroke center.Methods: All 44 patients with acute ischemic stroke from January 2017 to January 2018 were selected as the control group,and 83 patients with acute ischemic stroke from February 2018 to February 2019 were selected as the experimental group.Choosing quality management tools such as medical failure mode and effect analysis and root cause analysis to apply in the quality control of stroke center construction.Establish an improvement team,analyze the original thrombolysis process and sub process,conduct hazard analysis on the process,select high-risk links,and improve the high-risk links that need to be improved.Select abnormal record cases for root cause analysis,find the most root cause of delay for analysis and improvement.At the same time,prospective and retrospective research methods were used.The data of thrombolysis time(DNT),blood routine return time,time from admission to arrival of stroke team,time from arrival of stroke team to arrival of medication,mortality rate,neurological deficit score and average hospital stay were recorded.We use these indicators to evaluate the role of quality management tools in the construction of advanced stroke centers.Results: the mean time of thrombolysis decreased from 69.5 minutes in the control group to53 minutes in the experimental group,and the mean time of blood routine return decreased from31.5 minutes in the control group to 26 minutes in the experimental group.The risk of delayed thrombolysis reduced from 1706 to 409.The mortality rate and neurological deficit score in the experimental group were lower than those in the control group.The improvement is significant.Conclusion: In the construction of advanced stroke center,using appropriate quality management tools such as medical failure mode and effect analysis and root cause analysis can shorten the time of emergency thrombolysis,reduce mortality,improve the functional recovery of patients,be conducive to process improvement,reduce the risk of delayed thrombolysis,and promote the quality improvement of stroke center.At the same time,strengthen the team cohesion,effectively expand the regional influence of the hospital,and strengthen the regional construction of stroke prevention and emergency treatment.It is practical and reproducible and suitable for use in medical units.The cohesion of the team is strengthened,which is suitable for use in medical units. |
