Objective:Adenomyosis often occurs in women during their childbearing period.It is caused by the invasion of endometrial glands and stroma into the myometrium.The main clinical symptoms are progressive dysmenorrhea,which can be accompanied by the increase of menstrual volume,the prolongation of menstrual period,secondary infertility and other clinical manifestations.In this study,the pregnancy rate,cesarean section rate,preterm delivery rate,placenta implantation rate and postpartum hemorrhage rate of patients with adenomyosis were compared and analyzed by UAE and HIFU.Methods:From July 2014 to January 2017,we analyzed 103 cases of adenomyosis with clinical manifestations such as dysmenorrhea,menorrhagia,prolonged menstrual period and fertility intention or requirements in Weihai Municipal Hospital and Weihai maternal and child health care hospital.According to the inclusion and exclusion criteria,103 cases of patients with uterine artery embolization,97 cases of patients with uterine artery embolization,and 97 cases of patients with uterine artery embolization were included and excluded The natural pregnancy rate,cesarean section rate,preterm delivery rate,placental implantation rate and postpartum hemorrhage rate of patients with adenomyosis treated by embolization and high intensity focused ultrasound were summarized and analyzed in 6 months,1 year,1.5 years and 2 years after operation.The effects of the two methods on pregnancy rate,pregnancy outcome and delivery complications were discussed.Results:There were 41 successful pregnancies in the intervention group within 2 years,with a pregnancy rate of 39.81%;62 patients in the Haifu group within 2 years,with a pregnancy rate of 63.92%.The pregnancy rate in the intervention group was lower than that in the Haifu group,with a statistically significant difference(P<0.05).The cesarean section rate of intervention group was 31.71%,and that of Haifu group was 25.81%,there was no significant difference(P>0.05).The preterm birth rate of intervention group was 12.20%,and that of Haifu group was 11.29%,there was no significant difference(P>0.05).The placental implantation rate of intervention group was 9.76%,that of Haifu group was 0,and that of intervention group was higher than that of Haifu group(P<0.05).The incidence of postpartum hemorrhage in intervention group was 14.63%,and that in Haifu group was 1.61%.The incidence of postpartum hemorrhage in intervention group was higher than that in Haifu group(P<0.05).Conclusion:High intensity focused ultrasound is superior to uterine artery embolization in the treatment of adenomyosis with fertility intention.
