Background:With the recognition of the long-term immobilization hazards of critically ill patients and the importance of their quality of life and functional recovery,people have gradually realized that early mobilization of critically ill patients can help prevent or reduce the complications caused by immobilization and improve the prognosis of patients.Therefore,paying attention to the practice status of early mobilization of critically ill patients and the barriers and facilitators in the implementation process will help increase the development rate of early mobilization of critically ill patients and provide a basis for formulating feasible early mobilization protocol.Objective:This paper describes the current practice status of early mobilization in critically ill patients and discusses the barriers and facilitators of early mobilization in critically ill patients.Methods:This study is a descriptive study,using the convenience sampling method,from September 2020 to March 2021,179 patients in the intensive care medicine department of a tertiary A general hospital in Beijing were selected for investigation.Use self-designed patient general information questionnaire,clinical data questionnaire,and early mobilization observation record sheet for critically ill patients to collect early mobilization practice status of critically ill patients,including early mobilization development rate,first early mobilization start time,first early mobilization method,The duration of the first early mobilization,the number of mobilization during the ICU hospitalization and the duration of the early mobilization during the ICU hospitalization,whether adverse events occurred and the interruption of early mobilization,further analyzed the barriers and facilitators.Results:(1)The early mobilization development rate of 179 critically ill patients was 43.58%,and the median start time of the first early mobilization was 4.00(3.00,6.00)days.The early mobilization mode was mostly Level 3-bedside passive-passive wheelchair,ICU The median number of early mobilization during the period was 4.00(2.00,6.00)times,and the median duration of early mobilization during ICU was 262.50(131.25,435.00)minutes.There were no adverse events in 78 patients with early mobilization,but early mobilization were interrupted due to treatment needs or the absence of doctors.(2)The barriers for early mobilization of critically ill patients include:patient’s condition and treatment factors,accounting for 58.10%;insufficient human resources,accounting for 7.26%.The facilitators for early mobilization of critically ill patients include patient condition and treatment factors,accounting for 100.00%;adequate human resources,accounting for 94.87%.Conclusion:The early mobilization of critically ill patients in this study are well developed,but they still face many problems.The patient’s condition and treatment factors,as well as human resources,can be both barriers and facilitators for the early mobilization of critically ill patients.The patient’s condition and treatment factors are an important basis for assessing the early mobilization of critically ill patients.A reasonable allocation of human resources can promote the development of early mobilization for critically ill patients.
