| Objective: To investigate the clinical value of chitinase 3-like protein1 in patients with liver fibrosis and provide theoretical basis for the application of chitosanase-3-like protein 1 in clinical diagnosis.Methods: A total of 392 outpatients and inpatients in the Department of Liver Diseases of Hunan Provincial People’s Hospital from September2018 to December 2019 were collected.Fibroscan was used to determine the liver stiffness measure(LSM).According to LSM,the patients were divided into non-fibrosis group,fibrosis group and cirrhosis group.Detect the serum chitinase 3-like protein 1(CHI3L1)level,the liver function indexes(ALT,AST,TBIL,DBIL,TP,ALB)and fibrosis indicators(HA,LN,C-Ⅳ,PC Ⅲ)of all subjects.Draw the ROC curve of the diagnostic value of serum CHI3L1 level and LSM value in patients with liver fibrosis,and compare the diagnostic value of serum CHI3L1 level and LSM in patients with liver fibrosis.Part of the patients(n=114)in the cirrhosis group were selected and divided into cirrhosis compensatory group(n=20)and liver decompensated group(n=94).Draw the ROC curve of the diagnostic value of serum CHI3L1 level and HA and C-Ⅳ values in patients with liver fibrosis,and compare the diagnostic value of serum CHI3L1 level and fibrosis indicators in patients with liver cirrhosis.Results: The differences in CHI3L1 levels,LSM values,liver function indexes among the groups were statistically significant(P<0.05).There was no significant difference in the level of CHI3L1 in the non-fibrosis group with various liver function indexes and LSM values(P>0.05);There was no statistically significant difference between the serum CHI3L1 level and various liver function indexes in the fibrosis group(P>0.05),but the differences between CHI3L1 and LSM were statistically significant(P<0.05);The level of serum CHI3L1 in the cirrhosis group was compared with ALT and LSM and ALB,and the differences were statistically significant(P<0.05).The serum CHI3L1 level and the area under the LSM value curve were 0.841 and 0.717,respectively.The detection methods of the two were effective for liver fibrosis.The diagnostic efficiency of patients was high,and the diagnostic efficiency of serum CHI3L1 was slightly higher than the LSM value.Comparison of CHI3L1,liver fibrosis indexes between the cirrhosis compensated group and the decompensated group were both statistically significant(P<0.05).There was no statistically significant difference between the level of CHI3L1 in the compensation group of liver cirrhosis and various liver fibrosis indexes(P>0.05);The level of CHI3L1 in the decompensated cirrhosis group was compared with HA and C-Ⅳ,the difference was statistically significant(P<0.05),and compared with LN and PC Ⅲ,the difference was not statistically significant(P>0.05).Draw the ROC curve of serum CHI3L1 level and the diagnostic value of HA and C-Ⅳ values in patients with liver cirrhosis.The serum CHI3L1 level and the area under the curve(AUC)of HA and C-Ⅳ values are 0.812,0.633,0.480,respectively,indicating the serum CHI3L1 level It has high diagnostic value in the staging of liver cirrhosis.Conclusion: Serum CHI3L1 level has high diagnostic value for liver fibrosis. |
