Objective(1)Build an active health management platform of hypertension patients in community,according to the E-health literacy and self-management level of hypertensive patients in the community.(2)Discuss the influence of the platform on electronic health literacy,self-management level and blood pressure value of hypertension patients in community.Methods(1)From October 2019 to July 2020,four communities in Hengyang were selected by convenient sampling,and then 144 patients with hypertension who established health records in community were selected as the survey objects according to simple random sampling method.General information questionnaire,Chinese Eheals scale and self-management scale were used to investigate the status of active health management in 160 patients with hypertension.(2)With the help of Wechat Mini apps,a community active health management platform for patients with hypertension was designed and built,including three main modules of health knowledge,offline activities and hypertension self-management plan.(3)From September to November in 2020,two communities were selected as intervention group and control group by drawing lots and 30 hypertension patients with health records in two communities were selected as research objects by simple random sampling.The control group was given routine community hypertension nursing intervention,and the intervention group was given community hypertension patients active health management platform management.(4)SPSS 21.0 was used for statistical analysis.Mean±standard deviation,median and interquartile distance,constituent ratio(%)were used for descriptive analysis.Two independent sample t test,paired t test,chi square test,one-way ANOVA,multiple linear regression analysis and logistic regression analysis were used for statistical analysis.P<0.05 as significant difference.Results(1)The e-health literacy score of hypertension patients in community was skewed,with the median of 24 and the interquartile range of(18-29).Univariate analysis showed that there were significant differences in age,education level,occupation before retirement,personal monthly income,medical expenses,residence,blood pressure classification,preferred consultation way for physical discomfort and attitude towards network health information(P<0.05).Multivariate analysis showed that there were significant differences in personal monthly income,medical expenses and residence(P<0.05).(2)The score of self-management of hypertension patients in the community was(70.23 ± 11.14),indicating that patients’ self-management was at a moderate level.The score of risk factor management dimension was the highest.The lowest score was diet and sports management dimension.The results of single factor analysis showed that there were significant differences in six factors,including different occupation,economic situation,medical expense,blood pressure level,improper choice of counseling approach and the importance of acquiring knowledge of Network Health(P<0.05).Multiple stepwise regression results showed that the four factors of physical discomfort were the first choice of counseling(asking relatives and friends),age,blood pressure rating and education level(P<0.05).(3)After 3 months of intervention,the total score and application dimension score of e-health literacy in the intervention group were higher than those in the control group(P<0.05).The total score of e-health literacy in the two groups was higher than that before intervention,and the difference was statistically significant(P<0.05).(4)After 3 months of intervention,the total score of self-management level,treatment management and life habit management of hypertension patients in the intervention group was better than that of the control group(P<0.05).The self-management level of the two groups was higher than that before intervention,and the difference was statistically significant(P<0.05).(5)After 3 months of intervention,the systolic blood pressure of the intervention group was lower than that of the control group,and the difference was statistically significant(P<0.05).Systolic blood pressure and diastolic blood pressure in the intervention group and the control group were lower than those before the intervention,and the difference was statistically significant(P<0.05).Conclusions(1)The E-health literacy and self-management level of hypertension patients in the community still need to be improved,which is related to the patients′ awareness of active participation in health management and external environment of patients.(2)The active health management platform for hypertension patients is helpful to improve the E-health literacy and self-management level of hypertension patients in the community.
