Objective:To investigate the expression of epidermal growth factor receptor(EGFR),p16 and CD274 protein in hypopharyngeal squamous cell carcinoma and to analyze the correlation between them,and the relationship with the clinical and pathological parameters and prognosis.It can provide a evidence for individualized treatment of hypopharyngeal squamous cell carcinoma.Method:The histopathological specimens and clinical data of 60 patients with hypopharyngeal squamous cell carcinoma were collected,and the expression of EGFR,p16 and CD274 protein in hypopharyngeal squamous cell carcinoma was detected by immunohistochemical method.The correlation between the expression of the three proteins was analyzed by Pearson correlation.Using theχ~2test and Fisher’s exact test,the expression of the three proteins and the clinical pathological parameters of the patient were analyzed.The Kaplan-Meier method was used to calculate the survival rate of the patients and draw the survival curve,and the Log-rank method was used to test the significance of the difference in survival rate of each group.Cox regression model was used to conduct single-factor and multi-factor analysis of factors affecting prognosis.Results:1.In this group of 60 patients with hypopharyngeal squamous cell carcinoma,the positive rates of p16,EGFR and CD274 were:28.3%(17/60),75%(45/60),36.7%(22/60).2.The expression of p16 and EGFR was negatively correlated in hypopharyngeal squamous cell carcinoma(r=-0.662,P<0.001),and the expression of p16 and CD274 was positively correlated in hypopharyngeal squamous cell carcinoma(r=0.289,P=0.025),the expression of EGFR and CD274 was negatively correlated in hypopharyngeal squamous cell carcinoma(r=-0.280,P=0.03).3.Only the expression of CD274 is correlated with the patient’s N stage.The CD274 negative group is more likely to have more lymph node metastasis than the positive group(P=0.047);the age and gender of the patient between the EGFR,p16 and CD274 negative group and the positive group There were no significant differences in smoking history,T stage,N stage,clinical stage,and pathological grade(P>0.05).4.In patients with hypopharyngeal squamous cell carcinoma who received radiotherapy,the 2-year overall survival(OS)of the p16-positive group(17 cases)was 68.8%,and the 2-year local regional control(LRC)rate was 62.5%.,Better than 35.1%and 16.2%of the negative expression group(43 cases)(P=0.009,P=0.002);the EGFR negative expression group(15 cases)had a 2-year OS of 78.6%and a 2-year LRC of 71.4%,excellent 33.3%and 15.4%in the positive expression group(45 cases)(P=0.009,P=0.001)5.The three proteins are combined for survival analysis,and it is found that the EGFR+/p16-group and the EGFR+/CD274+group The prognosis of patients in the p16-/CD274+group is the worst,and the prognosis of patients in the EGFR-/p16+group,EGFR-/CD274+group and EGFR-/CD274+group is better.6.The expressions of p16 and EGFR are related to the prognosis of patients with hypopharyngeal squamous cell carcinoma,and EGFR is an independent prognostic factor for patients with local control.Conclusions:1.p16,EGFR and CD274 are all expressed in hypopharyngeal squamous cell carcinoma,and there is a significant correlation between the expression of the three proteins.2.The expression of p16 and EGFR are related to the prognosis of hypopharyngeal squamous cell carcinoma patients receiving radiotherapy.EGFR is an independent predictor of local control after radiotherapy.3.The combined detection of p16,EGFR and CD274 can provide more accurate predictions for the prognosis of patients with hypopharyngeal squamous cell carcinoma.
