| Objective:By reporting a case of a 14-year-old girl with primary hypothyroidism complicated by bilateral giant ovarain cyst in our hospital,We are aimed at providing the Van Wyk-Grumbach syndrome(early identification of juvenile hypothyroidism with bilateral giant ovarian cyst)with certain ideas and basis for the diagnosis and treatment,accumulating clinical experience,reducing the misdiagnosis rate,and avoiding unnecessary surgical interventions.Methods:Medical history collection,physical examination,laboratory examination were performed on the patient.Through preliminary identification and diagnosis,25 ug of levothyroxine sodium was given on each morning and was supplemented to 50 ug after 1 week of treatement.The syptoms,signs,laboratory and imaging results of the patient were followed up after 1 month of treatment.Combined with relevant case reports at home and abroad,analyze and summarize the pathogenesis,clinical features,diagnosis and treatment methods and prognosis of Van Wyk-Grumbach syndrome.Results:After 1 month of replacement therapy with thyroxine tablets,the patient’s edema,unresponsiveness and other symptoms improved significantly.The re-examination of thyroid function improved,and the levels of prolactin PRL and estrogen E2 decreased notably.The re-examination of abdominal ultrasound showed that the bilateral ovarian cystic masses were reduced compared with the previous ones.Conclusion:When juvenile hypothyroidism is accompanied by abnormal ovarian and sex hormone levels,the unified thinking of diagnosis should be taken as priority,and replacement therapy with levothyroxine tablets was given to avoid performing non-essential surgical treatment blindly. |
