ClInical Study On ACL Injury And High Tibial Osteotomy Guided By The Idea Of‘Tendon-bone Balance’(Jin Gu Ping Heng) Theory

Objectives:The study aimed to explore the role and importance of correcting skeletal structures in the treatment of knee anterior cruciate ligament injuries combined with varus knee based on the theory of "muscles and bones balance";high tibial osteotomy was performed to adjust the abnormality of knee joint skeletal structure,autogenous tendons had been used to reconstruct the anterior cruciate ligament;the clinical efficacy of the treatment of knee joint deformity combined with anterior cruciate ligament injury was observed and followed up,some new insights and ideas for the treatment of knee anterior cruciate ligament injury based on the theory of traditional Chinese medicine was then confirmed.Method:Retrospective analysis of 20 patients who underwent high tibial osteotomy and anterior cruciate ligament reconstruction from January 2017 to January 2020 in the sports injury orthopedics ward of Shandong University Hospital of TCM.The patients were divided into treatment group(A)and control group(B):A Group:Arthroscopic anterior cruciate ligament reconstruction+high tibial osteotomy group;Group B anterior cruciate ligament reconstruction group;functional evaluation at 6 months and 12 months after follow-up.The treatment group and the control group were followed up to observe the Lysholm knee function score,VAS score(visual analog score),and knee HSS score at 6,12 months before and after treatment;SPSS 20.0 was used for statistical analysis,the data of the treatment group and the control groupResults:(1)A total of 40 patients were included in the criteria,and there were 20 patients in the treatment group(tibia osteotomy+anterior cruciate ligament reconstruction),aged 32-50 years old,with an average age of 41.70±5.76 years old;There were 11 men and 9 females in the control group(anterior cruciate ligament reconstruction group),20 cases aged 33-50 years old,with an average age of 41.80±4.24 years old;12 males and 8 females.The average height of the treatment group was 164.65±6.34cm,the average height of the control group was 165.65±6.89cm,the average weight of the treatment group was 57.95±6.19kg,and the average weight of the control group was 56.85±6.37 kg.After the t-test,it was found out that there was no statistically significant difference in age,height and weight of each group.(Before treatment between the two groups and the control group,the average value of Lysholm in the treatment group was 53.10±4.32 and the average value of VAS was 6.97±0.80;the average value of Lysholm in the control group was 51.80±3.17,and the average value of VAS was 6.90±0.87.The patient’s gender,age,Lysholm index score before and after treatment and VAS index score before treatment were calculated using the social science statistical software package(SPSS20.0).The analysis result P<0.05,which is statistically significant,indicating that the treatment group and the control group The subjects in the two groups have comparable attributes.(3)Comparison within the group:The VAS scores of patients in the treatment group and the control group showed a downward trend with the progress of treatment time,6 months after treatment,12 months and the comparison before treatment,significantly decreased(P<0.01),indicating that both the treatment group and the control group can improve knee joint function;comparison between the groups:the difference in VAS scores between the treatment group and the control group before treatment is not statistically significant(P>0.05),which is significant In 6 months and 12 months,the treatment group improved significantly compared with the control group during follow-up,and the difference was statistically significant(P<0.05),indicating that the conventional treatment of pain in the treatment group was more effective than the control group.(4 Treatment group and control group Before treatment,the Lysholm score of the patients in the group was not statistically different(P>0.05).After 6 months of treatment,the Lysholm score of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).Treatment 12 Months later,the Lysholm score of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05);the comparison of the HSS scores of the knee joint between the treatment group and the control group before treatment showed no significant difference(P>0.05).After 6 months of treatment,the knee HSS score of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).After 12 months of treatment,the knee HSS score of the observation group was significantly higher than that of the control group.High,the difference is statistically significant(P<0.05)Conclusion:Follow the guidance of the theory of "muscles and bones balance",the symptoms of ACL-injury can be treated by the medial open tibial osteotomy.The short-term clinical effect is obvious.The mid-to-long-term curative effect needs further follow-up observation.
