| Objective:In this study,the effective compatibility of Astragalus,Aconite,and Tinglizi was used to form Yiqiwenyang Recipe.Combined with the pathological mechanism of chronic heart failure,through animal experiments and clinical studies,the effect on exercise tolerance of patients with chronic heart failure and the treatment of heart The mechanism of decay.Methods:1.Effect of Yiwenqiyang Recipe on exercise tolerance of patients with chronic heart failureThe selected 64 cases of chronic heart failure patients with heart function of grade Ⅱ-Ⅲ and age between 49-79 years were randomly divided into a control group and a treatment group.In the control group,32 cases were treated with conventional anti-heart failure western medicine,and the treatment group 32 cases were treated with yiqiwenyang recipe at the same time as conventional anti-heart failure western medicine,the treatment cycle is 8weeks.The brain natriuretic peptide(BNP),6-minute walk distance,left ventricular ejection fraction(LVEF),left ventricular end-systolic diameter(LVESD)and left ventricular end-diastolic diameter(LVEDD)were measured before and after treatment.2.Effect of Yiqiwenyang Recipe on mitochondrial oxidative phosphorylation complexⅤ in rats with heart failureSixty SD rats were randomly divided into blank group,model group,astragaloside IV group,western medicine(trimetazidine)group,low-dose Chinese medicine group and high-dose Chinese medicine group.The heart failure model was established by intraperitoneal injection of adriamycin hydrochloride.Astragaloside IV,western medicine(trimetazidine),low-dose and high-dose Yiwenyangfang decoction were used for drug intervention.On the 6th week of the experiment,the rats were subjected to echocardiography and plasma B-type brain natriuretic peptide(BNP)determination,and the left ventricular tissue of the rats was taken after the successful model was evaluated.then strictly follow the operation steps of the myocardial tissue mitochondrial kit instructions to extract rat myocardial mitochondria,and then follow the animal mitochondrial oxidative phosphorylation complex V activity quantification and detection kit instructions to determine the changes in complex V activity by spectrophotometry.Results:1.Clinical trials:(1)BNP: Compared with before treatment,the BNP of the control group and the treatment group were reduced,and the reduction degree of the treatment group was more obvious than that of the control group(P<0.05);(2)Cardiac function:Compared with the control group,the treatment group’s apparent rate,effective rate,and total effective rate increased significantly,while the inefficiency decreased significantly(P<0.05);(3)Cardiac color Doppler ultrasound: The LVEF values of the two groups increased compared with before treatment,and there was no statistical difference in the degree of increase between the two groups(P>0.05);the LVESD and LVEDD values of the two groups were lower than before treatment,the treatment group compared with the control group,the degree of decrease was more obvious(P<0.05).2.Animal experiment:(1)BNP:In the 6th week,the BNP value of the model group was significantly increased(P<0.05),and the BNP of the other groups was significantly lower than that of the model group;(2)Cardiac color Doppler ultrasound: Compared with the model group,the LVEF and LVFS of the other groups were significantly increased,and the LVESD and LVEDD were significantly reduced,the differences between two groups is significant(P<0.05);(3)The activity of oxidative phosphorylation complex V:Compared with the model group,the activity of oxidative phosphorylation complex V of the other groups was improved,and the difference was obvious(P<0.05).Conclusion:1.Clinical trials have confirmed that Yiqiwenyang Recipe can improve the heart function and exercise tolerance of patients with heart failure by increasing LVEF,6-minute walking distance,lowering BNP,reducing LVESD and LVEDD,etc.2.Animal experiments have proved that Yiqiwenyang Recipe can improve heart function by increasing the activity of myocardial mitochondrial subgroup oxidative phosphorylation complex V,thereby achieving the purpose of treating chronic heart failure. |
