| Objective: To observe the effect of Haungdi Nei Zhen for postoperative analgesia after total knee arthroplasty(TKA)and the effect of patients on functional rehab ILitation.Methods: A total of 60 patients with knee osteoarthritis(KOA)who were hospitalized in the Orthopedic Ward of Ruikang Hospital affiliated to Guangxi University of Chinese medicine from September 2019 to February 2021 were selected.The patients were numbered from 1 to 60 in the order of admission,and the patients included in the study were randomly divided into a drug group and a acupuncture group with 30 cases each using SPSS 24.0.The drug group was given intravenous injection of parecoxib sodium from the first day after surgery,twice a day;after 1 week,oral administration of celecoxib was changed to 200 mg twice a day.Acupuncture group on the basis of the drug group from the first day after the operation,accepted Huangdi Nei Zhen treatment with disposable sterile acupuncture needles for 2 weeks.According to the Huangdi Nei Zhen principle,find the pain points in the vicinity of the elbow joint of the contralateral upper limb of the patient’s surgical site,and ask the patient to appropriate flexion and extension of the knee joint as guidance cooperation.Record the visual analogue scores(VAS)of pain,knee motion(ROM)scores,aggregated locomotor function score(ALF)before and after treatment,and the level of serum C reactive protein(C-reactive protein,CRP),the postoperative use of additional analgesic drugs and adverse reactions as the evaluating indicators.Finally use the application software SPSS24.0 to analyze datas.Results: The study finally completed 59 cases,and 1 case fell off in the acupuncture group.The results showed that: 1.There was no statistical difference between the two groups of patients in such baseline data like:gender,age,height,weight,Bm I,course of disease and the background of preoperative medications etc,and they were comparable(P>0.05).2.VAS score:(1)The VAS score of the acupuncture group was significantly lower than that of the drug group on the 5th day after the operation.,which is significant statistically(P < 0.05);(2)On the 5th,7th,and 14 th day after surgery,the VAS scores of the accupuncture group were significantly lower than those of the drug group,and the difference was statistically significant(P<0.05).3.Knee joint range of motion:(1)On the 3rd,7th,and 14 th day after operation,the knee joint active range of motion in the acupuncture group was higher than that of the drug group,improved significantly,and the difference was statistically significant(P<0.05);(2)Passive range of motion of the knee joint: On 14 th day the passive motion range of acupuncture group was significantly improved compared with the drug group,and the difference was statistically significant(P<0.05).3.aggregated locomotor function(ALF)score: the acupuncture group completed the ALF test on the 7th and 14 th day after the operation,and the time it took to complete the test was significantly less than those of the drug group,and the difference was statistically significant(P<0.05).4.Serum C-react protein(CRP)level: The average level of CRP in the acupuncture group decreased more significantly on the 14 th day after operation,and there was a statistical difference(P < 0.05).5.Postoperative additional analgesic use: 9 cases in the drug group and 2 cases in the acupuncture group take the additional analgesic pills,the difference between the two groups was statistically significant(P < 0.05).6.Postoperative adverse reactions: The number of adverse reactions in the drug group was significantly more than that in the acupuncture group,and the difference was statistically significant(P<0.05).Conclusions: 1.Huangdi Nei Zhen can help relieve postoperative pain in patients with unilateral TKA,especially the improvement of pain during passive movement of the knee joint,combined with Huangdi Nei Zhen intervention,is better than simply using NSAIDs.2.Combined with Huangdi Nei Zhen to assist analgesia,it can significantly improve the active and passive range of motion of the knee joint after TKA,and effectively reduce the time used for the ALF test of knee joint function,which is more conducive to the functional recovery of patients after surgery.3.Compared with the simple use of NSAIDs,combined with Huangdi Nei Zhen can more effectively reduce the postoperative CRP level of patients and reduce postoperative stress response.4.Huangdi Nei Zhen for postoperative analgesia and functional rehab ILitation of TKA has a disciplines to follow in point selection,simple operation,good safety,and can reduce postoperative adverse reactions. |
