| Objective:The aim of this study was to screen frailty in elderly patients with T2DM,and analyze the relationship between the distribution of TCM syndrome types and clinical variables,and analyze the relationship between frailty and related clinical variables.Material and method:We collected inpatients with T2DM in the department of Gerontology,Nanjing Hospital of Traditional Chinese Medicine from March 2020 to January 2021,and classified the included patients with TCM syndrome differentiation according to Guiding Principles for Clinical Research of New Chinese Medicines(Trial)(2012 edition)and Combining clinical practice.Screened the patients of frailty,and they were divided by the Frailty Phenotype Scale which is recommended by the ICFSR into group of non-frailty,group of pre-frailty and group of with frailty.We collected clinical data of all the patients,including age,gender,height,weight,BMI,duration of T2DM,FPG,2hPPG,HbAlc,TC,TG,HDL-C,LDL-C and other data.In the end,the relationship between the types of TCM syndromes,frailty and various clinical indicators of the included patients of T2DM was analyzed.Results:A total of 209 elderly patients with T2DM were included in this study,and 38 patients with frailty were detected,with a total detection rate of 18%.Compared with the group of non-frailty and pre-frailty,the group of frailty have a higher average age,a longer duration of T2DM,higher HbA1c and lower HDL-C,and the difference are statistically significant(P<0.05).Elderly patients with T2DM have the highest detection rate of frailty in the blood stasis syndrome,and the lowest detection rate of the Yin deficiency and heat syndrome.The average age of patients with Yin deficiency and heat syndrome is the youngest,and the average duration of T2DM is the shortest.The average age of patients with blood stasis syndrome is the oldest.The average age and duration of T2DM of the yin-yang deficiency syndrome is the closest to the average age of the sample population.In the logistic regression analysis,age,duration of T2DM,HbAlc and HDL-C can enter into the regression equation of frailty(P<0.05).These four are the influencing factors of frailty.Conclusion:1.There is a certain difference in the detection of frailty among the various TCM syndromes in elderly patients with T2DM.The average age of the Yin deficiency and heat syndrome is younger,and the duration of T2DM of this syndrome is shorter.The duration of T2DM of the yin-yang deficiency syndrome is longer,and the average age of the blood stasis syndrome is older.2.Blood stasis syndrome in elderly patients with T2DM is one of the risk factors affecting frailty.3.In elderly patients with T2DM,age,duration of T2DM,HbA1c,and HDL-C are the factors for frailty. |
