Study On The Correlation Between NLR,FIB And FAR Before Treatment And The Diagnosis And Prognosis Of Cervical Cancer

Objective:To investigate the value of neutrophil lymphocyte ratio(NLR),fibrinogen(FIB)and fibrinogen to albumin ratio(FAR)in the diagnosis and prognosis of cervical cancer(CC).Methods:181 patients with early cervical cancer(cervical cancer group)who were treated in our hospital from November 2009 to November 2019 were selected as the research objects,and 152 healthy women(healthy group)who were examined in the physical examination center of our hospital were selected as the healthy group Lymphocyte count,fibrinogen,albumin,NLR,FIB and FAR were calculated.The age and diagnosis of the patients were recorded.According to the pathological report,the classification,lymph node metastasis and differentiation degree of cervical cancer were recorded.The receiver operating curve(ROC)was used to analyze the indicators between groups of cervical cancer,and the best cut-off value was determined,Objective to analyze the value of systemic inflammatory markers in the diagnosis and prognosis of cervical cancer.Results:Compared with the control group,the levels of NLR,FIB and FAR in cervical cancer group were significantly increased,and the differences were statistically significant(all P<0.05).There was no significant difference in NLR,FIB and FAR in cervical cancer group(all P>0.05).The levels of NLR,FIB and FAR in cervical cancer group were significantly different in pathological stage(all P<0.05).There was significant difference between FAR and lymph node metastasis in cervical cancer group(P < 0.01);There was no significant difference in NLR and FIB levels in lymph node metastasis(all P>0.05).ROC curve analysis results: the area under the line of NLR in cervical cancer group was 0.799,95% CI was 0.752-0.841,NLR cut-off value was 2.13,sensitivity was 69.06,specificity was 84.21;In cervical cancer group,the area under the line of FIB was 0.872,the 95% CI ranged from 0.832 to 0.906,the cut-off value of FIB was 3.05,the sensitivity was 87.85,and the specificity was 75;In cervical cancer group,the area under the line of FAR was 0.929,the 95% CI ranged from 0.896 to0.954,the cut-off value of FAR was 0.06,the sensitivity was 96.69,and the specificity was 73.03.The 8-year survival rate of cervical cancer group with NLR>2.13 was lower than that with NLR≤2.13(P<0.05);The 8-year survival rate of FIB > 3.05 was lower than that of FIB ≤ 3.05,and that of FAR>0.06 was lower than that of FAR≤0.06,but there was no significant difference(P=0.268,P=0.716).NLR combined with FIB in the diagnosis of cervical cancer: the area under the line was 0.894,95% CI was 0.861-0.928,sensitivity was 86.19,specificity was 77.63,the differences were statistically significant(P<0.05);NLR combined with far in the diagnosis of cervical cancer: the area under the line was 0.952,the 95% CI range was0.930-0.974,the sensitivity was 94.48,the specificity was 86.84,the differences were statistically significant(P<0.05);FIB combined with far in the diagnosis of cervical cancer: the area under the line was 0.962,95% CI was 0.944-0.980,sensitivity was 93.37,specificity was 88.82,the difference was statistically significant(P<0.05);NLR,FIB combined with far in the diagnosis of cervical cancer: the area under the line was 0.967,the95% CI range was 0.951-0.984,the sensitivity was 90.61,the specificity was 91.45,the difference was statistically significant(P<0.05).Conclusion:NLR,FIB and FAR of before treatment have diagnostic significance for FIGO staging of cervical cancer.FAR has diagnostic significance for lymph node metastasis.NLR,FIB combined with FAR has high sensitivity in the diagnosis of cervical cancer,which can be used as the basis for clinical diagnosis and is helpful for early diagnosis and treatment.High preoperative NLR is an independent prognostic factor for the progression and survival of cervical cancer.
