| Objective:This study is mainly based on elderly patients with angina pectoris.Through retrospective analysis of blood biochemical indicators and coronary angiography results,the relationship between the platelet lymphocyte ratio(PLR)and the degree of coronary artery stenosis was studied relevance and its predictive value.Materials and methods:A total of 155 cases of elderly patients with angina pecto ris in the Department of Cardiology of Liaoning Provincial.People’s Hospital fro m October 10,2018 to October 10,2020 were collected retrospectively.All the ad mitted patients completed the coronary angiography examination and completed the r elevant blood within 24 hours.Biochemical test,the degree of coronary artery steno sis was assessed by Gensini score of coronary angiography.According to Gensini score,the enrolled patients were divided into low score group(Gensini≤30,n=99)and medium-high score group(Gensini>30 n=56).Collect clinical case data of all in cluded patients.Baseline data include: age,gender,weight,smoking history,histor y of hypertension,history of diabetes,etc.The 24-hour blood biochemical data inclu de: white blood cell(WBC),platelet(PLT),lymphocyte(LM),neutrophils(N),Creactive protein(CRP),low density lipoprotein(LDL),high density lipoprotein(HDL),cholesterol(CHOL),triglyceride(TG),fasting blood glucose(GLU),alanin e aminotransferase(ALT),creatinine(Cr),hemoglobin(Hb),etc.The statistical softwar e SPSS24.0 was used to process and analyze the data,the independent sample testwas used for the measurement data conforming to the normal distribution,the ran k sum test was used for the measurement data not conforming to the normal distr ibution,and the chi-square test was used for the count data.Incorporate variables with P<0.1 into the multivariate logistic regression analysis(increase the number ofsamples to facilitate the analysis of risk factors),use spearman to analyze the cor relation between PLR and the Gensini score of coronary stenosis,and plot the relati onship between PLR and the Gensini score of coronary stenosis Receiver Operating Characteristic(ROC),determine the Area Under Curve(AUC),and evaluate its predicti ve performance.P<0.05 considered the difference to be statistically significant.Result:1.Univariate analysis showed that there were significant differences in diabetes history,fasting blood glucose,smoking history and PLR between low Gensini score group(n = 99)and medium and high Gensini score groups(n = 56)(P < 0.05).PLR of patients with medium and high Gensini score was significantly higher than that of patients with low Gensini score.There was no significant difference in gender,age,history of hypertension,neutrophil count,hemoglobin,lymphocyte count,C-reactive protein,D-dimer,low density lipoprotein,high density lipoprotein,total cholesterol,triglyceride,alanine aminotransferase and creatinine between low Gensini score group(n = 99)and medium and high Gensini score group(n = 56)(P > 0.05).2.The history of diabetes,smoking,PLR,fasting blood glucose and total cholesterol(P < 0.1)were included in the multivariate logistic regression analysis.The results showed that smoking,diabetes and PLR were independent risk factors for Gensini score of coronary artery stenosis(P < 0.05).Spearman correlation analysis showed that PLR level was positively correlated with Gensini score(r = 0.318,P <0.05).3.The area under the curve was 0.691(95% Cl: 0.607-0.774).The cut-off value of PLR was 133.59,with the highest diagnostic efficiency,sensitivity of 89.3% and specificity of 56.7%.Conclusion:1.Diabetes history,smoking history and PLR are independent risk factors for coronary artery stenosis.2.PLR was positively correlated with the degree of coronary artery stenosis.With the increase of PLR,the degree of coronary artery stenosis was gradually aggravated.When the cutoff value of PLR was 133.59,the sensitivity and specificity of PLR were 89.3% and 56.7% respectively.The results suggest that if PLR > 133.59,the degree of coronary artery stenosis may be more serious.After further expansion of data and improvement of clinical research,it can guide clinicians to take relevant clinical intervention measures as soon as possible to prevent further progress of coronary artery stenosis and improve the prognosis of patients. |
